Driver had passed the Notice to stay in truck sign at least 2 times earlier in the day. Instructions to all personnel if they witness any non-employee out of their vehicle instruct them to get in and stay in vehicle.
Working on crusher with trackhoe, got between crusher and trackhoe attachment, and hoe operator moved the control stick by bumping it. That caused the hammer attachment to fall and strike the employee.
EE STATES HE WAS LOADING A HOLE AND THE HOSE BECAME STUCK IN SOME LOOSE ROCK. HE WAS LIFTING ANDTWISTING THE HOSE WHEN HE HEARD HIS SHOULDER "POP"
