Showing all 12Employee fainted for approx 1 minute. Regained consciousness and stated he improperly taken blood pressure medication, was transported by rescue service for Dr evaluation. Due to heart condition, pacemaker installed later by personal physician, 37 days off.
Employee was getting into loader seat holding onto door. A strong wind caught door, closing it suddenly causing employee to strike his head on window latch inside cab of loader causing laceration to head.
Employee was holding rope used to reposition dredge. Rope broke, causing clevis pin to strike his left hand, breaking a finger.
Lifting 5 gallon bucket of sand. Lumbar strain. Initial Dr. evaluation stated RTW - full duty - later went to light duty.
Employee was assisting belt replacement. Belt pulled in opposite direction causing pain to right shoulder.
Pain to groin area.
EE MINER STEPPED OVER DISCHARGE PIPE ON DREDGE. SURFACE WAS SET DUE TO LEAK THAT WAS BEING REPAIRED. MINER TRIPPED ON WET SURFACE, FALLING BACKWARDS STRIKING LOWER BACK ON PIPE, CAUSING CONTUSION TO LOWER BACK.
Miner was grading haul road with d-6 cat. when backing up, a piece of 4" metal pipe caught in track, flipped up and struck side of cab, throwing sand and debris into left eye.
MINER WAS PULLING CLASSIFIER SCREEN ACROSS PALLET WHEN HE FELT A SHARP PAIN IN LOWER BACK. HE WAS STOOPING DURING THIS TASK. REPORTED & DIAGNOSED 4/21/03.
29 YR OLD MALE WAS USING STILSON PIPE WRENCH TO TIGHTEN PIPELINE CHAIN CLAMPS. HE EXPERIENCED PAIN IN LOWER BACK THE NIGHT AFTER WARD.
EE WAS LIFTINMG A PUMP BY HAND WHEN HIS LEFT SHOULDER POPPED. PHYSICAL EXAM REVEALED POSSIBLE TORN ROTATOR CUFF LIGAMENTS.
