CMP Mining, LLC · Struck against a moving object

EE WAS OPERATING A S-CAR HAULING COAL. HE RAN IN A HOLE IN THE MINER FLOOR WHICH CAUSED THE SEAT BACK TO HIT HIM IN THE LOWER BACK. HE WORKED UNTIL 7-33-02. HE WENT TO THE DR. AND BEGAN MISSIN G WORK ON 7-23-02. THE REPORT WAS FILED LATE DUE TO THE REASONS I HAVE NOTED.
