Employee was walking from his loader to his personal vehicle when he slipped on some ice & snow and twisted his right ankle. No treatment was given at the time. On 1-28-11 employee said he was still having some pain in his foot. It was found to have a fracture in his right foot.

Employee was replacing the worn ball joints on Cat truck #26.3010 (775D). He was using a ball joint separator and 5 pound sledge hammer. When he was driving the separator in, it came loose (the separator) and hit him on the back side of his forearm just above his wrist, on his right arm. He was treated for the abrasion (6 stitches).

EE was lifting a 5 gal bucket of oil, filling a vertical turbine motor. He said he felt a warm sensation in the groin area. The dr said that it is a muscle pull.

Employee was cutting out the hammer bolts to out Primary Impacter crusher (with oxy-acetylene torch). He was driving the bolts out with a b&O hammer or small sledge. He swung the hammer and felt a pull in his lower back.

Employee was wiping down the inside of his loader. When he was cleaning aroung the air vents in the overhead A/C unit, his right hand came in contatct with the A/C fan. He was cut on his middle and ring fingers (the A/C unit was running at the time).

While removing the back plate on f-5 conveyor, the 3/4" plate was unbolted. EE was working with another ee and together they pulled the plate from the bolts. the plate fell back and pinched left little finger against tailpulley guard. this is reportable because ee had a splint put on finger.

HE WAS STRIPPING MATERIAL FROM HIGHWALL AND IT HAD CONTAINED A POCKET THAT WAS NOT VISIBLE. THE MATERIAL SLID OFF AND UPSET THE TRACK-HOE AND SHOVED IT ABOUT 100 FT. VICTIM WAS WEARING HIS SEAT BELT AND WAS TRAPPED FOR ABOUT 2 HOURS AND 25 MINUTES.

EMPLOYEE WAS PUTTING ZIP TIES ON ANOTHER EE'S HARD HAT TO HELP SECURE HIS CUTTING GOGGLES. HE PUT ONE OF THE ZIP TIES ON AND USED A STANLEY POCKET KNIFE TO CUT OFF THE EXCESS ZIP TIE WHEN THE KNIFE CAME TOWARDS HIS LEFT THUMB HE CUT IT @ THE 2ND KNUCKLE ON THE BACKSIDE OF HIS HAND.

PULLING UP TO THE LOADER IN THE PIT TO GET LOADED & DID NOT SEE THE LARGE ROCK & RAN OVER IT WITH BACK TIRES OF THE HAUL TRUCK WHICH JARRED THE HAUL TRUCK & THE OPERATOR.
