Employee was walking from his loader to his personal vehicle when he slipped on some ice & snow and twisted his right ankle. No treatment was given at the time. On 1-28-11 employee said he was still having some pain in his foot. It was found to have a fracture in his right foot.

EE was stepping out of the motor control building when he stepped down onto the catwalk. He lost balance and rolled his right ankle. Area was well lit; catwalk was clean with no obstructions.

EE was working in a manlift basket when he descended to the ground and was walking away from the manlift, when he slipped in the ice and fell on his back. Injured his right wrist.

Employee was replacing the worn ball joints on Cat truck #26.3010 (775D). He was using a ball joint separator and 5 pound sledge hammer. When he was driving the separator in, it came loose (the separator) and hit him on the back side of his forearm just above his wrist, on his right arm. He was treated for the abrasion (6 stitches).

Employee was lifting, tugging & pulling on 6" water lines, trying to connect them with dresser sleeves. He strained his lower back, ruptured a disc.

EE was walking down the steps. As he was stepping down, he partially missed a step and rolled his right knee/ankle.

EE was lifting a 5 gal bucket of oil, filling a vertical turbine motor. He said he felt a warm sensation in the groin area. The dr said that it is a muscle pull.

Employee was cutting out the hammer bolts to out Primary Impacter crusher (with oxy-acetylene torch). He was driving the bolts out with a b&O hammer or small sledge. He swung the hammer and felt a pull in his lower back.

ee was driving a komatsu haul truck, hit a hole in the road and he was holding onto the grap bar and his weight on his left wrist caused a sprain.

Driving a haul truck on the pit floor which is uneven, rocked the haul truck sideways and injured the driver's back (it started to hurt).

Employee was wiping down the inside of his loader. When he was cleaning aroung the air vents in the overhead A/C unit, his right hand came in contatct with the A/C fan. He was cut on his middle and ring fingers (the A/C unit was running at the time).

While removing the back plate on f-5 conveyor, the 3/4" plate was unbolted. EE was working with another ee and together they pulled the plate from the bolts. the plate fell back and pinched left little finger against tailpulley guard. this is reportable because ee had a splint put on finger.

EMPLOYEE WAS CLIMBING ON THE TRUCK WHEN HIS RT. FOOT SLIPPED OFF THE STEP AND HIS LEG STRUCK THE STEP CUTTING HIS RT. LEG.

HE WAS STRIPPING MATERIAL FROM HIGHWALL AND IT HAD CONTAINED A POCKET THAT WAS NOT VISIBLE. THE MATERIAL SLID OFF AND UPSET THE TRACK-HOE AND SHOVED IT ABOUT 100 FT. VICTIM WAS WEARING HIS SEAT BELT AND WAS TRAPPED FOR ABOUT 2 HOURS AND 25 MINUTES.

EMPLOYEE WAS PUTTING ZIP TIES ON ANOTHER EE'S HARD HAT TO HELP SECURE HIS CUTTING GOGGLES. HE PUT ONE OF THE ZIP TIES ON AND USED A STANLEY POCKET KNIFE TO CUT OFF THE EXCESS ZIP TIE WHEN THE KNIFE CAME TOWARDS HIS LEFT THUMB HE CUT IT @ THE 2ND KNUCKLE ON THE BACKSIDE OF HIS HAND.

EMPLOYEE WAS GREASING F-8 CONVEYOR. EE STOPPED TO CHANGE THE CARTRIDGE, PLACED GREASE GUN ON CONVEYOR, OPENED NEW TUBE OF GREASE. WITH GREASE TAB IN HAND HE REACHED FOR FALLING GREASE GUN. AS HAND CLOSED, THE TAB CUT EE'S RT. MIDDLE FINGER.

5' ELECTRIC HEATER WAS BEING MOVED THRU A LOW HEAD CLEARANCE AREA, NOT GIVING ENOUGH ROOM FOR PROPER LIFTING PROCEDURES.

EE WAS USING A LONG HANDLED SCREWDRIVER TO PRY OUT A SEAL IN THE BOTTOM OF VSI CRUSHER. SCREWDRIVER SLIPPED CAUSING EE TO HIT SIDE OF CRUSHER WALL WITH HIS RIGHT THUMB. THIS CAUSED THE EE S TH UMBNAIL TO BECOME DISLODGED FROM THUMB.

EE WAS CHANGING TRANSMISSION FILTER ON TRUCK. FILTER CARTRIDGE WAS EXTREMELY TIGHT. EE WAS PULL-ING ON FILTER WITH WRENCH AND HURT HIS BACK. E.R. ROOM STATED HE HAD INFLAMMED, PULLED/TWISTED MUSCLES IN HIS BACK.

WALKING OUT OF THE 7' CRUSHER OIL SHANTY CARRYING 5 GAL OF FUEL GOING DOWN A SMALL RAMP OF MATERIAL, LOST FOOTING & TWISTED LEFT ANKLE & KNEE. WEATHER CONDITIONS: COLD IN THE 20'S, SUNNY SKY, NO SNOW ON GROUND. EE SLIPPED BUT DID NOT FALL.

EMPLOYEE & CO-WORKER WERE WORKING OUT OF THE MANLIFT BASKET. WHILE EMPLOYEE HELD A BEAM THE CO-WORKER WELDED IT IN PLACE. A SPARK FLEW FROM THE WELDING INTO EMPLOYEE'S RIGHT EYE. HE WAS WEARIN G PRESCRIPTION SAFETY GLASSES. HE KEPT RUBBING RIGHT EYE, CAUSING MORE DAMAGE TO THE EYE.

EE WAS OPERATING THE BOOM TRUCK. EE BENT DOWN TO CHAIN A SKIRT BOARD. RIGHT KNEE POPPED WHEN HE STOOD BACK UP.

RAIN HAD CAUSED HAUL ROAD DETERIORATION. THE RAIN ALSO CAUSED POOLING OF WATER IN THE LOADING AREA. THE HAUL TRUCKS HAVE DRAIN HOLES IN THEIR BEDS, SPEWING ADDITIONAL WATER ON THE ROAD. EE SAI D HIS NECK & LEFT SHOULDER WERE STIFF & SORE FROM OPERATING THE HAUL TRUCK IN THESE CONDITIONS.

PULLING UP TO THE LOADER IN THE PIT TO GET LOADED & DID NOT SEE THE LARGE ROCK & RAN OVER IT WITH BACK TIRES OF THE HAUL TRUCK WHICH JARRED THE HAUL TRUCK & THE OPERATOR.
