Employee tripped over a water hose as he was attempting to close a water valve. The valve was a 4 inch gate valve with a locking handle. The handle had a sharp edge on it. Employee received a laceration to his right shin that required sutures.

Showing all 21Employee was attempting to remove a broken bolt from a bucket pin on a front end loader. The chisel slipped as he was striking it with a hammer and the hammer struck his left thumb. The employee recieved a nondisplaced small distal tuft fracture of the left thumb.
Employee was cutting a piece of conveyor belting with a utility knife, when the knife slipped and he cut his right, front thigh.
Section of high wall collapsed, causing a rock to come through the front windshield of front end loader. Employee recieved a laceration to his left thumb. Did not require treatment until 08/14/2008, when a piece of glass was removed.
Employee was walking along a pathway from the primary crusher when he slipped in some mud. He felt a burning sensation in his left knee, but continued working and did not report the accident untill the next day 01/10/2008. Employee was sent to local hospital on 01/10/2008,where he was diagnosed with a sprain to his left knee.
Employee was inside the cargo compartment of a river barge, when he slipped and twisted his ankle. He suffered an strain injury to the right ankle.
Employee had cut a piece of steel with an oxygen & acetylene torch. When he picked the piece of steel up to move it, he failed to notice a jagged edge. When he placed the piece of steel over his shoulder to carry it, the jagged edge lacerated his right ear. Employee was treated at local emergency room, recieved 6 sutures and released to return to full duty.
Cutting steel with a chop saw, a piece of steel went around his safety glasses and into his eye.
Employee was operating a roadgrader, he had his left hand on the doorframe, the door was locked in the open position. The door came loose from the holding mechanism and shut on the employee's left index finger. Employee sustained a fracture to the left index finger above the distal joint. Doctor splinted the finger and OK'd the employee to return to work.
Strained lower back while lifting a piece of metal.
Employee was using a box blade knife to cut a piece of conveyor belting during a repair to the conveyor belt. The knife broke causing him to cut his right thumb. Required surgery to repair a severed tendon. During mandatory drug testing due to the accident, he tested positive for Meth and Marijuana and was subsequently terminated.
While chipping at a weld, a piece of slag bounced off inside of the safety glasses and went into his left eye.
Employee lifted a piece of 1/2 inch steel plate prior to cutting it into sections with a gas torch. Employee demonstrated that he bent at the waist and lifted and moved the plate. Employee was released to restricted duty by the Dr., however he failed to return to work. The Physicians diagnosis was "low back strain, sciatic features to L - resolving".
Employee was welding, employee states he does not recall a single event that would have caused the injury. Employee states that he thought he had contracted "pinkeye", until he went to Dr. and foreign objects were removed. Employee did not seek medical attention, or notify company of injury until 05/02/2005.
Employee was climbing a step ladder while carrying a container of engine oil. EE slipped from the second step and fell about 2 feet to the floor below. Employee struck his nose during fall and sustained a laceration.
Employee was helping with the movement of an electrical control box. After box was moved employee complained of a sore back.
Stepped off the crusher and twisted right ankle.
DURING A BLOW BAR CHANGE IN THE IMPACT CRUSHER, EMPLOYEE WAS STANDING NEXT TO THE ROTOR. THEY HAD CHANGED ONE SET OF BLOW BARS AND WERE GETTING READY TO MOVE THE ROTOR TO THE NEXT SET. CO-WORK ER MOVED THE WEDGE OUT. VICTIM SLIPPED AND FELL ONTO THE ROTOR. THE IMBALANCE OF THE ROTOR PULLED HIM DOWNWARD AND CRUSHED HIM.
WAS LOADING WIRE ROLL STAND INTO LOADER. THE STAND SLIPPED OUT OF BUCKET & HIT FOOT JUST ABOVE STEEL TOE. BROKE BONE IN FOOT.
EE WAS INSTALLING FAN ON LOADER. IT WAS NOT UNDER POWER. THE FAN SPUN SLIGHTLY, WHICH CAUGHT HIS LEFT THUMB BETWEEN FAN AND SHROUD. LACERATION OF LEFT THUMB AND CUT THE TENDONS.
EE WAS MOVING A POWER TRANSFORMER ONTO THE BARGE WITH TWO OTHERS. THEY LOST THEIR GRIP & IT FELL ON HIM MOMENTARILY. THE INJURY WASN'T BOTHERING HIM UNTIL 7/26/02 IT BECAME UNBEARABLE.
