miner walked near head pulley of the trummel discharge belt when plant was starting up and was struck by an unexpected rock falling from the belt. the rock struck the miner in the left foot. the steel toe in the boot was crushed causing crushing injuries to the big toe and the index toe.

Showing all 7Employee changing the feeder oil on power screen jaw crusher. While taking off the large feeder pan for inspection, the pan slipped from hands and injured employee's finger.
Employee was using a come-along rake to clean stone out from under a conveyor belt, felt a strain in his lower back and hip area.
Confirmed Standard Threshold Shift from audiogram re-test.
Mechanic was working in the engine compartment of a track excavator. The hood was up and latched. While he was re-setting the hood in place a gust of wind picked the hood up and it came down on his head.
EE strained his right wrist while working around finish plant on 1/8/07. He did not report any problem until 1/15/07, nor was there no report of any incident until 1/15/07. EE worked his full shift & had his wrist checked after work. It was then determined that he strained it and put on restricted duty.
A CONTRACTOR WAS MOUNTING A NEW STEELE CHUTE ON A CONVEYOR HEAD SECTION. THE CHUTE WAS PRYED FREE & SHOT UP IN THE AIR WHICH PINCHED THE CONTRACTORS LEFT HAND. HE HAD 3 STITCHES IN ONE FINGER & 2 STITCHES ON THE IN THE OTHER.
