Sometime between the hours of 1200 am and 6:00 am an unknown male between the age of 25 and 35 committed suicide. The victim used a self-purchased rope and entered onto the mine site, where he secured the rope to a railing on the screen and walked below and committed the act of suicide.

Employee was tightening a bolt with a wrench when the wrench slipped off the bolt and the employee's elbow struck a piece of metal on the equipment being worked on.

Splicing a conveyor belt on the radial stacker. As employee was cutting the belt the belt ripped and knocked the employee off the belt. Employee fell approx. 10 feet to the ground.
