Showing all 8Injured employee was in the process of dumping a load of coal off of his tractor/trailer when the trailer turned over, which in turn turned the tractor over. Injured employee's neck was lacerated during turn over. What caused the laceration was undetermined.
Employee picked up wash down hose to clean coal out of crusher. He was using his right hand to twist the nozzle to the on position. When the sudden burst of water pressure came on, the employee did not have a secure hold on the nozzle and the hose. This resulted in the employee's wrist being put in an awkward position, therefore, spraining his right wrist.
Employee tripped over a 3" toe plate while walking backwards giving hand signals to a crane operator. Employee fell on the existing catwalk bruising his knee, ribs, and shoulder. He was OK until a few days ago, decided to go back to the doctor on 9/15. Doctor put him on modified duty, but no modified duty is available, therefore it's a lost time.
Employee came into foreman's office and reported that his back was hurting and that he was going to seek medical attention. He informed his foreman that he had injured his back while pushing up loads of coal in the yard.
DURING MAINTENANCE TO A HEAD ROLLER SHAFT A BELT SCRAPER SLIPPED AND SMASHED THE INJURED'S RIGHT INDEX FINGER RESULTING IN A FRACTURE AND LACERATION.
EE CLOSED A VALVE ON HYDRAULIC SYSTEM, CAUSING A BUILD UP OF PRESSURE TO A POINT ABOVE HYD HOSE PRESSURE LIMIT. THE HOSE BURST, HIGH PRESSURE OIL STRIKES ARM AND BODY CAUSING BRUISING AND ARM LACERATION.
EMPLOYEE WAS DIRECTED TO SHOVEL IN AND AROUND SURFACE STOKER PLANT FEEDER AREA. EMPLOYEE BEGAN TO SHOVEL COAL PILE SPILLAGE AND TWISTED LOWER BACK.
