Operator was hauling mud in a 980H loader when EE hit a bump in the haul road and received a possible pinched nerve in EE's neck. EE is seeing a physical therapist for pain relief. No time lost, no light duty restrictions, still working job of injury.

Showing all 38Miner was closing a valve and their hand slipped as they were tightening the valve. Miner lost balance and they attempted to catch themselves as they fell. Miner injured their left elbow when they fell.

Placing punch plate in place, plate slipped and smashed left ring finger between screen and boot. 6 stitches were received at Enumclaw ER. IE was immediately released to return to work.
Had just completed using electrical testing rod. Stood up. Went to put on hard hat with rod still in hand. Rod went under safety glasses and poked eye.
Employee was welding using Arc welder. When finished he removed his hood. He then stood up. As he was standing up he bumped the rod causing it to turn on. While it was on he received a flash burn to his eyes. Employee did not go to the doctor until 3/21/2015
Two employees were removing a screen when the injured employees crow bar slipped and hit him in the hip.
Employee pinched his finger between a cone and the wedge he was using to secure the cone still while he was turning the bolts on the cone. EMPLOYEE DID NOT REPORT THE INCIDENT UNTIL THE NEXT DAY. HE DID NOT SEEK OR RECEIVE MEDICAL TREATMENT UNTIL 5/24/2012. I FOUND OUT ABOUT THE MEDICAL TREATMENT WHEN THE DOCTOR CALLED ME DIRECTLY ON 5/25/2012.
A snap ring was coming off of a radial stacker tire. The employee was trying to repair/reset the snap ring when it popped off the tire, hitting the employee in the right ring finger. The employee needed stitches for the laceration on his finger
Strained his lower back when lifting a piece of a machine. Employee did not report this accident officially or visit to doctor. We were made aware of his trip to the doc on 9/2/11 when we received workers comp information from L&I. Employment was termed for unrelated issues on 8/17/11, employee saw doc for first time on 8/20/11. No return to work due to layoff, not injury.
No Miners involved. Early morning on 7/21/2011 private vehicle driving on private roads swerved off road, ran through 3 fences and into mine property. Vehicle hit berm at edge of pond. Both driver and passenger were taken from scene by ambulance. Before mine opened for the day. Was immediately reported to MSHA.

While removing bearing on the head pulley of the feeder belt a piece of steel splintered off and lacerated employee's left forearm.
Employee was hitting a part with a rubber mallet. He was holding the part with his left hand. He hammered down catching his left little finger, smashing it.

Employee was climbing up to get into a piece of equipment when he slipped down and hit his side on the equipment.
Employee was working in a fuse box and did not properly shut off the power to the fuse box first. Caused an arc that caused flash burns to employees hands.
Employee was using a torch to cut a conveyor frame. When he cut away one portion of the conveyor frame the other portion where he was tied off to began to collapse and slide down a hill. As he tried to jump free, his safety harness pulled him into a conveyor cross member, striking his back and right side. He was taken to the local hospital by ambulance.
Employee was getting out of a truck, jumped to ground, acute lumbar sprain.

While employee was lowering a lift cylinder onto a loader, employee tried to line up the cylinder. His finger got caught/smashed between the cylinder and the loader. No property damage, could lose a portion on his right ring finger.
Employee was unloading concrete blocks out of the back of a truck when his left thumb got squeezed between the chains used to lift the ecology blocks. Pressure caused a cut in his thumb.
One employee was lifting a steel plate with a forklift; injured employee was standing on the ground spotting and giving direction. Forklift got caught on the steel plate underneath the one being lifted. The steel plate being lifted slipped off of the forks and hit the injured employee in the chest and on the chin. Caused bruising to the ribs and slight fracture of the collarbone.
Employee dropped a 4 foot long piece of metal on top of left foot. Did not go to the doctor at the time. Went to doctor the next day, no treatment given. Continued to hurt, went back to doctor, off work from 12/3/06 to 12/6/06, back to work 12-6-06 went on light duty til 2/6/06, then off work til 2/26/07, then back full time
INDIVIDUAL WAS CUTTING A BELT WITH A UTILITY KNIFE.HE WAS CUTTING WITH A MOTION TOWARD HIS BODY WHEN THE KNIFE RELEASED FROM THE BELT. HE STRUCK HIS LEG WITH THE BLADE.
Employee was climbing down from a conveyor when he stepped down he twisted his knee.
Employee was dismounting a forklift when he slipped & grabbed the roll bar. Wrenched his left shoulder. Initially it was just reported as an incident. Didn't seek medical attention until 6/27/05(from 2200602400092 2/5'd). Returned to work & then went off 7/18/05 with same injury - went for surgery. Returned 1/9/06
EE WAS WALKING UNDER A CONVEYOR WHILE PAINTING IT. HE STEPPED ON SOME UNEVEN GROUND AND FELL, HURTING HIS ANKLE.
