Employee was driving CAT 777 rock truck with an air ride operator's seat. According to the operator, the seat had plenty of air at the beginning of the shift. The air ride seat lost air throughout the day. The employee hit a rough spot in the road and the seat bottomed out causing them to jar lower back and tailbone area.

Employee was replacing conveyor chain in a truck dump feeder. The chain was being pulled out by a hoist truck. The flight keepers were unknowingly wore out and 1 flight fell out of the chain and struck the employee on the left hand and knocked 2 more flights out of the chain as it was being pulled out.

Employee was servicing a 988G loader 129' from a highwall. A root cluster with dirt and rock fell from the highwall and, upon impact, projected the dirt and rock into the haulroad towards the loader. A rock 2" x 3/4" came through the engine compartment and glanced off of the employee's head. Employee received 3 staples to close the laceration.

Employee was assisting in dropping a transmission into a 993K loader. They bent over to move some hydraulic hoses out of the way and felt a sharp pain in the left arm and shoulder. However, the employee stated they had recurring pain in this area for over a week and could not pinpoint this injury to this particular instance. This is still under investigation.

Employee was operating an empty rock truck back to the pit via the right contour haul road and met a loaded rock truck travelling in the opposite direction. Employee pulled to the side of the road to let the truck by. The rock truck traveled over an irregularity in the road at a high rate of speed, jarring the employee into the right armrest of the operators seat.

Employee was exiting a D11R. EE stated that EE's right foot was on the last step on the push arm as EE was lowering EE's left foot down to the ground. EE lost EE's balance on some frozen material and twisted EE's knee as EE tried to regain EE's balance.

The employee slipped and fell while walking across the parking lot at the end of shift. This resulted in an injury to right arm. On 03/17/2018 the employee went to the doctor and found that the employee in-fact fractured arm. Employee reported this to supervisor on 03/18/2018. The employee has not missed any time due to this accident as of this date.

The operator was notified 7/8/16 of a claim for occupational pneumoconiosis for the affected employee. The employee was terminated doe to a reduction in work force on April 15, 2014.

The operator was notified of a claim for Occupational Pneumoconiosis after mining has ceased and the affected employee was laid off. The operator wishes to report the Illness Claim and terminate the affected miners employment since mining equipment has been removed and mine entrances are sealed.

A fire occurred in a stockpile where high sulfur coal had been stored. The fire occurred due to air pockets existing within the stock pile due to loose compaction of coal which caused the coal to spontaneously ignite. No personal injuries were inflected, or equipment damages occurred from the fire. No persons were endangered from the fire.

A roof fall was discovered at approximately 8:40 AM between spad 75637 and 75663 in the number 1 entry (left return). Dimensions of the fall were 120' long x 18' wide x 6' high. The area was bolted with 6' fully grouted bolts. Location of the fall was 1,600' outby the section dumping point.

Exiting IT14 loader, when he stepped down onto ground with his left leg under him, his knee buckled causing him to fall to the ground. Employee continued to work in his normal occupation until 7/27/15 and then started missing time.

EE picked up one side of the top roller while another ee was lifting the other side and when they turned to install the top roller in the rail, ee felt a sharp pain in his left calf muscle. EE continued to work in his normal occupation until 6/15/2015 and then started missing time.

Employee was driving a mantrip from the outside to the section, when a rock fell from the top and struck in the head / shoulder area knocking his hard hat and glasses off of him. He was sent by Ambulance to SMH and was granted a full release. He informed Mine Management that due to the injury he received on 10/21/14 that he would be going off work on 3/16/15.

The employee sought medical treatment at SRMC ER for irritation. The eye was irrigated and the employee was given an antibiotic ointment.
