Roof fall in #8 entry on 001 section due to slick and slide it was app. 60' long, 16' wide, 8' high. Partly covering S/C
Doing weekly run, found rock fall in old works in travel way.
Was driving mantrip the wheel caught a rock causing the knee on the steering wheel to spin around and then catching EE's hand hard.
While unhooking chain on front of scoop he put his left hand down to balance his self putting his finger's in between the ram and the back of the scoop bucket. The scoop operator sucked the ram in to loosen chain pinching his hand.
While realigning a come-a-long he slipped and hit his side and ribs on belt structure.
While making a belt splice in the #5 belt, employee was holding a 6 ft board under the splice. When co-worker was pounding in the splice, the hammer slipped hitting employee's finger.
We believe that due to a lack of cable bolts in an intersection which had slick & slides in the top it caused the top to weaken & fall.
After shutting the belt off the belt man was crossing over belt when he lost his footing twisting his knee
When weekly examiner was traveling the bleeders with MSHA they discover a intersection had fell above anchorage in IE2 panel. The air way had not been blocked but alternet rout of travel was put in to play.

Two employees were building a crib tower. One employee was tossing cribs to the other, while the victim was picking up a crib the other employee tossed one striking the victim's hand causing a laceration to the victim's pinky finger.
A roof fall occurred in the #4 entry of the Southeast mains section, MMU 001-0. The fall occurred approximately 80 feet inby S/S 509. The fall was approximately 40 feet in length, and 20 feet of the area that fell was unbolted. The roof supports in the area that fell out were 4' fully grouted resin bolts. The height of the fall is unknown, but is above the anchorage zone.
Employee was towing a skid of belt from the surface to the section when the chain holding the belt came unhooked and the belt slid off of the skid. The employee then tried to lift the belt back onto the skid by himself, resulting in a hernia.
In intersection 1 block inby last open cross cut roof had slick and sides fell 9-10' high.
Backing up a trailer piece of waterline caught slide up hit him in face.
Employee was pulling miner cable, when pulling the cable his back muscle went into a spasm.
Roof bolter helper (injured employee) was pulling slack cable off the roof bolter cable reel. The roof bolter operator accidentally started to wind the cable reel while the bolter helper was pulling it, causing the cable to jerk the bolter helper and strain his back.
Employee was advancing discharge line for a face pump. While pulling it he twisted wrong and pulled his back out of place. Employee continued to work until 7/25/12 when he reported the accident and sought medical attention.
While bolting he had his hand on roof bolt. Swung head in hitting his hand on canopy.
While putting waterline in he was pounding two joints together with a crib he missed the end of the water line and pinched the end of his middle finger.

While the bolting crew was bolting a corner, the section boss was watching them bolt. The bolting crew told him to move from where he was because of loose rock. He didn't and a rock and head coal mix 4" thick, 4'X4' fell a distance of 48", hitting him on the back and head.

EE did not use proper precautions to move roof bolter. He did not keep his body and head inside of the machine when moving the machine.
While bonding a intake stopping he used his bare hands to put b-bond on a block stopping, he was offered a pair of gloves. He choose not to use them.
While setting post employee was reaching down to pick up a wedge and another post hit him on the fingers.
EE was operating left side of bolter, installing a roof bolt, when a hose busted and sprayed hydo oil in face and chest area.
Rock fell out of top.
He caught middle finger on right hand in sheave wheel while pulling belt.
Caught left foot between scoop bucket & p/c.
Employee came to work and turned in a doctor's note saying he hurt his back but did not report it to his boss or to me when it happened
He was a bolter operator off side operator was advancing lift, when off side operator misplaced foot bolter tire ran over it.
Roof fall in spad 959 60' outby in #1 heading. Measured roughly 15' high, 12' wide, 40' long. Horseback slip fell out 8 feet of rock.
