Showing all 35Employee twisted knee on right leg when slippery conditions caused employee to lose footing on CO9 chute while washing out CO8 conveyor.
Employee was using a pry-bar to dislodge a rock at the chute at CO5. The pry-bar slipped and smashed EE's little left finger between the pry-bar and another rock. After employee was evaluated by a physician it has been determined EE's finger is fractured.
Employee was attempting to bend light back out after it had been bent previously by someone else. Pipe wrench slipped and hit other hand that was on the boom injuring EE's finger.
pulling up piece of cat walk with chain hoist, employee had stopped applying pressure the neck of hoist on end of chain broke allowing hoist to strike employee in face. employee was taken by supervisor to concentra received 3 staples and 2 stitches facial contusion, laceration of lip, contusion of head, scalp laceration was given work restrictions.
Was adjusting strap on pipe slid and made contact with employee's left hand. Employee was seen at E R on 9/10/20 missed work on 9/11/20 and returned to duty on 9/12/20
EE stepped out of truck to retrieve ticket slipped caught foot between rail on scale and step of truck resulting in lumbar strain ee was put on restricted duty 02/28/2019 I was notified of this accident on 02/28/2019 by ee HR director ee is an over the road dump truck driver employed by mine
operating the 914 loader doing cleanup hit rough spot jamming back
Employee wrote that they were cutting a belt with the utility knife when it slipped off of the belt and punctured leg. Employee went to St. Elizabeth Health care and received four stiches, returned back to work Monday when scheduled. No lost time occurred. Accident occurred at 9:10 pm employee started work at 2:00pm.
Employee crouched on one knee cutting a recoil spring. Spring popped causing employee to jump back on left foot spraining foot.
Employee was cleaning out chute of the C 0 3 belt line while doing that his back froze up and started to hurt
The injured said that he was cleaning up tunnel #4. He had finished hosing the tunnel and was dragging the hose with him as he left. He tripped on a rock and fell forward into the superstructure of the conveyor in the tunnel. The injured fell face first into a piece of angle iron on the conveyor, striking his mouth. He broke 8 teeth and his lower jaw.
Injured was exiting the water truck and slipped from the top step because of material built up on steps. Injured slipped approximately 4 feet and twisted his left ankle when he landed.
The injured miner was attempting to move a rock with another miner who had wedged a jackhammer under the rock. The miner removed the jackhammer and the rock fell back to the ground smashing the injured miner' s right hand, fracturing his middle and ring finger.
Customer truck had arrived at the quarry and was being loaded his first load of the morning. Notified his employer that he was having chest discomfort. His employer sent him to mine office.
IW was working near primary screening operation. A large rock came off screen and hit IW on head. He was wearing hard hat and the suspension system in hard compressed and when it did it lacerated IW scalp
IW was assisting co-worker in preparing steel for welding, when a piece of rust came off of metal and went into left eye.
IW was attempting to free a cable by kicking the cable
IW was cutting down tree with chainsaw, the tree fell into second tree causing first tree to kick back, hitting chainsaw and making the chainsaw hit employees right leg.
IW was closing drawer on file cabinet, drawer hung up, won't slide in and IW lifted right side of drawer and felt something "pop" in her left arm.
While using a spud bar to clean out the syntron at Plant #1, the spud bar slipped causing the injured worker's left index finger to be pinched between the bar and the roof.

IW's left little finger was smashed between two gears.
Injured worker was in the proximity of another maintenance worker, who was grinding on steel plate.
IW walked into a protruding piece of steel that was stacked on a service truck.
Employee was exhibiting signs of breathing difficulty; he was having difficulty standing and moving/slurring his speech. BP was taken: 220/124. No injury on job just an onset of these conditions. High blood pressure was a pre-existing condition/he was taken to the nearest hospital by Hilltop personnel and admitted. He was released 4/19/07 w/ a rtw release of 4/23/07.
IW WAS REPLACING SCREENING MEDIA AND SLIPPED DOWN DISCHARGE CHUTE

IW was stndng on the lft side of the trlr. The drvr chked the side mirror, he saw IW begin to walk away from the trlr. The drvr then noticed a pu truck approaching on the rt side. The drvr looked to see that the pu was not pulng in front of him. He then began to pull fwd. While pulng fwd he rechckd his mirrors and saw IW falling to the grnd. The drvr stopped the truck.
Employee was loading a steel plate onto the flatbed welding truck with a helper. The plate twisted and fell onto his finger, mashing and cutting the last knuckle area of his finger.
EMPLOYEE WAS CLEANING MUD OUT OF THE PRIMARY CRUSHER'S FEEDER FLOOR UTILIZING A SHORT SPUD BAR AND A 10 LB SLEDGE HAMMER. HOLDING BAR WITH HIS LEFT HAND AND HOLDING THE HAMMER WITH HIS RIGHT HAND, EE WOULD STRIKE BAR WITH HAMMER TO DISLODGE MATERIAL. EE MISSED BAR WITH HAMMER HEAD AND INSTEAD PINCHED RIGHT INDEX FINGER BETWEEN END OFBAR AND HAMMER.
EE WAS FUELING TANKS ON FUEL TRUCK AND HAD FINISHED ONE COMPARTMENT. WHEN CLOSING LID, HE PINCHED RT MIDDLE FINGER AT NAIL BETWEEN LID AND MAIN BODY. EE REPORTED IT IMMEDIATELY BUT IT REQUIRED NO FIRST AID AT TIME. LATER ON EE SAW MORE BLOODDRAINED FROM UNDER NAIL. EE DID NOT MENTION THIS ON VISIT UNTIL 5-23-02, DR'S OFFICE DID NOT SEND ANY INFO UNTIL 5-28-02.
EE WAS DRIVING STOCKPILE TRUCK AT #1 PLANT WHEN COMPLAINED OF LOWER BACK PAIN. HE BELIEVES THAT MOTION/RIDE OF TRUCK CAUSED HIM TO STRAIN LOWER BACK. EE HAD SEEN A DOCTOR BUT NO CONCLUSIVE RES ULTS WERE MADE. ON 6-4-02 EE CALLED IN & SAID THAT HE WOULD NOT BE IN (BACK WAS HURTING) & GOING TO DR'S OFFICE. DR SAYS HAS ABNORMALITY CALLED SPONDYLOLYSIS. MISSED 1 DAY.
