A SCREEN DEVELOPED A HOLE IN IT. WE TOOK THE OLD SCREEN OUT THEN WENT AND GOT A NEW SCREEN AND LIFTED IT WITH THE SKID STEER TO EE, WHO WAS ON THE CAT WALK. HE LIFTED THE SCREEN OFF OF THE SKID STEER BUCKET AND BALANCED IT ON THE HANDRAIL. HE MENTIONED THAT SOMETHING POPPED IN HIS ARM. I CALLED AND RECEIVED MORE HELP WITH THE SCREEN AND EE RAN THE LOADER THE REST OF THE DAY

Showing all 5Using a 4" electric grinder to grind a metal frame. Small speck of metal in the eye was not found by the miner until he was at home later that evening. He tried to flush with water then went to urgent care the next morning. He was wearing safety glasses and a face shield and can only speculate that this in how it happened. Doc removed object & prescribed ointment.
EE was cutting a conveyor belt for a splice with a razor knife. At the end of the cut the knife slipped off the belt and contacted his knee, it made a small cut but due to the depth of the cut we decided he should o to Urgent care. We cleaned the cut and applied gauze & took him to Urgent Care where he received three stitches. He returned to work the next day.
Employee fell but no one observed his work being performed or his fall. Employee died.
WE WERE DRILLING HOLES IN BELTING THAT FIT AROUND GRIZZLY FEEDER. THE CUTTER TOOL WAS ACTING UP, NOT DRILLING HOLES LIKE IT NORMALLY DOES. EE GRABBED UTILITY KNIFE TO MAKE HOLES BIGGER. HE WAS HOLDING THE BELT STILL WITH HIS LEFT HAND. AS HE WAS CUTTING HOLES, THE KNIFE SLIPPED OFF AND CUT HIS LEFT HAND. 5 STITCHES REQUIRED.
