IE was assisting in taking a bad piece of cable out of the slope hoist cable. They were picking the cable up to move the cable over. As the IE raised up from picking cable up, IE felt a pain in the lower to middle back. IE did not go to the Dr until 3/16/19 & Dr could only find a possible strained muscle in IE's back. IE started missing on 3/18/19.

IE was carrying old belt structure out of the slope drive house to get rid of it from where they had stacked it on the surface after changing it out for new structure in the slope. EE and another employee lifted the structure & ee felt pain in lower back & left buttock. All that they could find wrong at the Dr. was possibly a pulled muscle.

IE was throwing roof bolter cable over against the rib so it would be out of the way for haulage an the MN 5 section. As IE stepped forward IE felt a pain in the left knee.

IE was setting up to start loading with the CM. IE had adjusted IE's tubing for ventilation & as IE walked outby towards the end of the CM a rock hinged off of the last row of roof bolts & knocked the tubing down. The tubing knocked IE down & the rock flipped over onto IE's left ankle which caused the injury.

IE was assisting in the building of an overcast. As the crew lifted an I-beam IE felt a pain in the right shoulder. IE continued to work until 1/30/19 and that is when they started missed work due to this incident that happened on 10/21/2018.

IE was traveling North pulling supplies with a locomotive. As ee was approaching the 18E switch, the locomotive went East into the switch because someone had left it open. IE's Locomotive struck a parked manbus that was unoccupied. IE starting missing on 9/26/18 due to this incident.

IE was traveling North in a locomotive as the tail Locomotive to the #6 Loco that was pulling supplies. As the #6 loco went into the 18E switch that was left open by someone the #6 loco hit the manbus that was left unattended in the 18e switch & the IE that was driving #9 loco at the tailend was injured as well. IE started missing on 9/25/18 due to this incident.

I.E. was assisting in changing belt rollers in the slope. As I.E. released the come-a-long, left middle finger was caught in the chain. It cut the tip of the left middle finger off. I.E. went to Dr. & was treated. I.E. came back to work on next scheduled shift & has not missed any work due to this accident.

EE was using a cross-over to go make a seal on the examination route. When they stepped off of the cross-over they slipped & cut head on the hand rail of the cross-over. EE received 3 staples to close the wound but returned to work on the next shift. No work was missed due to this incident.

IE was washing down the floor in the #1 drive house on the surface belts. IE stepped off of the stair step onto the concrete floor & felt a pain in lower back. IE did not miss work until 06/13/2018. Dr. hasn't found anything wrong yet but IE hasn't returned to work due to back hurting.

IE was helping on the longwall recovery face. EE was unhooking the shield to walking shield hoses. EE was standing between the shield & the crib when a 2'X4"X2"-4" thick rock fell on EE. EE had 1 small laceration on forehead that required stitches & another small laceration on the back of the head. No broken bones were found by the Dr.

IE was helping on longwall recovery. IE had been sitting on the mine floor taking hoses off of shields. The material in the J-Cribs had mixed with the mud on the mine floor causing burns on IE's buttocks & back of legs. IE started missing on 5/23/2018.

IE was helping with the longwall recovery. The material in the pumpable cribs had mixed with the mud & water on the recovery face. Employee had gotten this chemical on the back of legs & buttocks. It caused chemical burns to the skin. The Dr kept EE off for 5 days due to the burns.

Face chain had broken & they were trying to put it back together. They had a com-a-long holding the tensioner motor down. The operator engaged the tensioner but put too much pressure on it & the com-a-long chain broke & hit the I.E. in the nose & right side of face. It fractured their nose & safety glasses pushed up & got stitches above the eye.

We have contracted GMS to help with set-up of our new longwall. One of their employees went to the return to use the bathroom & didn't have any toilet paper so they used a knife to cut a shirt to use instead. They cut the left thumb trying to cut the shirt to use for toilet paper & got 3 stitches. They returned to work their next shift & missed no time.

Crew was pulling gate box cable slack on the trollies. The cable dog got caught & broke. When the cables came down it hit the I.E. in the right ankle causing a small cut which required 3 stitches. I.E. did not miss any work due to this incident.

I.E. was unloading vent tubing from a scoop. I.E. tripped over the track rail & landed on the other rail with right knee. I.E. was wearing knee pads. I.E. was taken to the Dr. & only bruising was found. I.E. was released to return to work but stated that I.E. just felt that I.E. couldn't return yet.

I.E. put a bolt on the boom of bolter after drilling hole. When ee moved boom the bolt hung on the frame of the bolter & popped up, hitting ee in the mouth. It moved one of ee front teeth forward causing ee to get dental work. The I.E. has not missed any time due to this incident.

EE was working off of a ladder on the 18 east section. When dismounting the ladder, slipped and fell backwards, hitting lower back on a bucket of plaster located in the area of the ladder. EE finished shift with soreness/tightness of lower back. EE reported to next scheduled shift but requested to follow up with hospital the following morning.

EE was working at the 17 East/MN3 header. EE was working with another employee was turning the belt off to remove a rock from the chute. Before the belt was turned off, EE attempted to remove the rock and EE's hand was caught between the rock and the belt. The rock was removed and the hand was released from the belt and rock and EE was brought outside for an ambulance.

EE was the MN2 take-up shoveling and washing with a hose. EE was shoveling a large amount of gob and when lifting it to the belt, felt a sharp pain to lower back. EE continued to shovel and stated aggravated back even more.

EE was fire-bossing in 0-gate when EE noticed a burning/throbbing feeling in the right lower leg from the top of foot to the knee. EE finished shift and when EE returned for the next scheduled shift, EE reported that EE needed to go to the doctor, that EE could not perform EE's duties as a fire-boss.

EE was removing a slider on the 18 east section when EE felt a strain to lower back. EE proceeded to go to get a grease gun to grease the miner, and upon returning the grease gun, EE's back pain became more intense. EE informed the section foreman that EE needed to seek medical help for back strain.

EE was attempting to move a 2 inch air pump from 59 cross-cut on the belt line to the sump.EE thought the air line and depressurized and attempted to move out from the belt area. When moving the pump, the air line came off of the pump, which caused EE to lose balance and fall into the old structure and rollers, causing a sprain to right wrist and right knee.

While changing scoop batteries an attempt resulted in the batteries positioned improperly on the forks of the scoop. The operator lifted the batteries and prepared to back against the rib. The injured employee was standing at the back of the scoop. EE flagged operator to stop. While getting jumper out of the way batteries slid off end of forks falling onto employees left foot.

EE and the evening belt crew were making a belt splice on MN2. EE was releasing the belt clamps from side of the belt, while the other side had been released, which resulted in left thumb being struck by the belt clamps. EE was taken to the hospital for evaluation, which resulted in gluing of the affected area, along with 2 stitches placed.

During Longwall setup procedures, two service representatives from shield manufacturer were installing yield valves on shields. The deceased was positioned behind leg jacks installing yield valves on tilt jack, while second representative began to install yield valves on leg jack. Plugs from both jacks were removed causing the shield canopy to collapse crushing the victim.

Bolter was spotting what EE thought to be pot in 6R Face on East Mains Section. Hole was drill with no issue and bolt push up to set, but operator pushed plate against roof and began to rotate causing sparks to ignite a small pocket of CH4 that was outgassing from above seam. Flame burnt less than 5 seconds.

Employee was loading bolt supplies onto bolter from scoop. The top fell out in front of pinner and 'operator side' bolt person went to inch tram controls to back pinner outby. The scoop was parked 3' (estimate) from bolter with bucket in elevated position. The injured employee was caught between scoop bucket & pinner causing a roofbolt to puncture leg just below right knee.
