Two employee were unloading a pallet of wires. The hurt employee dropped the pallet causing it to hit his left wrist. Employee suffured left wrist sprain. This Accident was originally report online August 4, 2006; however, for some reason it never went through the system.

Employee suffered lower lumbar strain due to improper lifting/twisting of core samples. The samples weight is near 100(lbs). Employee was turning around core sample to read the label for inventory. The root cause of the injury were samples stacked to high. The contributing factors to the injury were improper lifting/twisting heavy objects. However, no damage or impairment.

Applying belt dressing to head pulley of pug mill conveyor, employee inserted arm behind guard and sleeve was caught by conveyor. Employee suffered arm dislocation, fracture, and abrasions before he was able to hit conveyor stop button.

Discharge conveyor stopped so he was turning rotor to look at the belt when his hand (right thumb)got cut by rotor blade.
