While mixing copper sulfate, some of the solution splashed onto employee's face and under his safety glasses into his left eye. He was released full duty. In reviewing the file it was recogonized he was given presciption medication for comfort which makes this reportable.

At the 5-5200 south bench, a fall of ground from the back fell approximate size 10' by 10' by 20' long. The area was not open to personnel only remote equipment. The fall was approximately 178 feet from where personnel had access. A miner thought the rib had failed and reported it. The next morning the mine foreman went to look it over and determined the failure was from the back.

While driving a bolt in the back with a jackleg drill, bolt slipped off driver and bolt fell out of hole striking employee's upper lip. 4 sutures were used to repair.

While unloading a mounted 26.5R25 Tire from service vehicle to spare rack, tire & wheel assembly hit the spare rack and came off Bead Hook being used to lift assembly. It fell to ground pushing injured into side of service truck. Resulting in contusion to posterior portion of left shoulder/rib area and fractured ribs.

Two mechanics were traveling on the surface haul road in a kubota buggy. When the operator attemtped to stop, the buggy began to slide in the loose gravel; the passenger reacted by putting his leg out to keep the buggy upright and received a fractured lower leg.

Employee was walking down a slope of a mountain and tripped over a piece of sagebrush causing him to trip. Resulting in a sprained left ankle.

Additional ground support was being installed in an area that was in need of it. Two 8' split set bolts had been installed and more holes being drilled. Small material started "dribbling" at the brow so they backed off a safe distance to observe it. After about five minutes the brow & more material fell exposing two 8' swellex bolts that appeard to have been installed at an angle.

Employee was tightening the water swival onto a 10' NT drill rod when his 24" pipe wrench slipped, causing him to hit the drill deck with his right hand, spraining his wrist.

The EE was drilling a hole with a jackleg, when a rock came out of the back through the 4"X4" wire mesh. The small rock hit the EE's hand/finger that he was holding the jackleg with.

The EE was hanging electrical cable and a pull bottle with a cable hanger. The scissor deck truck he was standing in moved, causing the cable to be pulled out of his hand. When this happened, the cable hanger (attached to the cable and pull bottle) cut the palm of his hand.

The worker was moving & dumping precip from a cart in the refinery around 7 a.m. The precip cart (which has 2 full pans on it weighs around 1300 pds) was being moved when the worker pulling on the cart felt something snap in his shoulder.

While helping electrician cut tape off of a pipe on a jumbo, the box knife slipped off the tape & sliced into his finger as he was holding the cable.

On 2/15/2007 an employee reported an incident occurring back in August of 2006 where he hyper-extended his knee and then two more incidents at work and one at home. Employee continued to have problems with knee and eventually had surgery to repair a ligament. Employee lost 4 days and had 9 restricted days.

Employee was jacklegging in the 4-4650 when his safety glasses became dirty. He then took then off to clean. While cleaning them, a rock fell hitting the muffler, which made the rock bounce back striking him in the eye. At the clinc he was prescribed a cream and placed on light duty.

Employee was attempting to attach a chain around a refuge chamber door to a forklift fork. As the fork was being boomed out his right arm was inadvertantly pinched between the fork & the door.

The jumbo operator leaned a 5' x 10' wire up against the rib to paint it for bolting. He had the wire too vertical and when he walked away to get another wire, the wire fell forward and struck him on the arm causing laceration/puncture to his left arm elbow.

The employee leaned a cume-along against the rib. He thought it was secure, When he walked away, the cume-along fell and struck him on the right shoulder.

Employee was operating a 30 ton haul truck. He hit a bump in the road which caused his cap lamp to come off the bracket on his hard hat. When his cap lamp came off it swung down in front of his face. It struck a front tooth chipping the back side.

The employee was drilling a hole with a jackleg. The steel abruptly stopped turning causing the jackleg & hoses to rotate as the machine (Jackleg)rotated the employee was still holding on to it. This caused him to strain his shoulder.

7/11/06 employee reported to her supervisor that her knee had been hurting for several days. She beleived pain was result of twising & turning at work. The next day, 7/12/06, she went to the doctor and was placed on restricted duty.

A fall of ground occurred in the 4-5 450 heading where roof bolts were in use. This was above the anchorage point of the bolts. It is not know when this occurred, however, upon discovery it was immediately reported to MSHA and a 103k order was issued.

Miner was walking down 5 north when he felt a shootingpain in his lower back. The miner had picked up a prill pot earlier in the day and thought his back was reacting to that. He was taken to the doctor adn was placed on restricted duty.

Ee was removing a bolt from the ware plate on the bucket off the 988 loader. The bolt started to spin, so he tried to hold it with his hand. The bolt soun cutting through his glove and into his hand. He was taken to the medical facility, received stitches and released to full duty.

EE was operating a Getman utility truck. EE was taking supplies to a heading down off of 3 haulage. The driver tried to avoid hitting 2 bumps in the road. The truck bounced up and down, EE had his seat belt on, and pulled the muscle in his lower back.

EE has been diagnosed with silicosis. A conclusive diagnosis was given on 4/10/2006 by Dr. Cahill at University of Utah, pulminary clinic.

While mounting a 950X20 tire, picked up tire to rotate and install on equipment, lifted to mount Right Frt tire, felt a pop in back, and could not bend over afterward.

While fueling loader, he stepped off and slipped on the frame of the loader, causing his ring and pinky fingers on his right hand to bend backwards. It was not reported until the next day. He was taken to the doctor and placed on restricted duty.

Employee was bolting in the 4-4850. Employee used a scaling bar with a bearing plate to hold up the side of the wire in order to drill a hole. The scaling bar kicked loose and the end with the bearing plate hit his forearm.

While working on drill around 9:30 pm I felt a pain in my back (upper left side). I continued to work but after awhile I could not lift even an empty tube. The pain had started to go down my left arm & halfway up my neck. I informed driller & we went to hospital.

Employee was going to dump a bucket of muck into the muck bay with a 2 yard LHD. The forward pedal stuck causing the mucker to keep going forward. The employee struck his elbow on the cab. The mining company did not know of any light duty from the employee until 1-26-06.

Employee was drilling a hole with a jackleg drill. When pulling the drill steel and machine out, the jackleg came off the drill steel which made the drill pivot down and smashed the employee's finger.

EE slipped on a muddy rock, to stop his fall he put his hand out to brace himself against the rib. The EE jammed his finger in the action of stopping his fall. The EE did not report the accident until 1-16-2006 then he sought care after that.

The ee was operating the 2 yard lhd in reverse. The 2 yard lhd hit a hole in the road which caused the lhd to bounce. He bounced striking his head on the remote box that was mounted in the operator's compatment.

THE EMPLOYEE THOUGHT THERE WAS AN EYELASH IN HIS LEFT EYE. HE RUBBED IT. THE IRRITATION DID NOT GO AWAY. HE WENT TO THE DOCTOR TO HAVE IT LOOKED AT AND A PIECE OF METAL WAS REMOVED. HE WAS GIVEN A PRESCRIPTION FOR ANTI-BIOTICS AND RELEASED TO FULL DUTY.

Employee was back dragging in spiral 3-4750 sill. He was finishing ramping the sill when boulders from top of the muck pile rolled underneath his bucket which caused the mucker to bounce up and sideways. Employee's head hit the top of the FOPS/ROPS or the remote box causing pain in his neck.
