H B Potash LLC · Struck against a moving object

Crew was fully pre-tasked prior to start of shift. Two scrapers were operating in a push pull fashion. The lead scraper operator lowered the can into the dirt prior to the rear push scraper connecting and the lead scraper came to a full stop. Rear scraper hit the back of the lead scraper and lead scraper operator sustained a left shoulder strain.

Fire reported by Cont. @ 5:05 pm involving surface bldg being demolished.Safety arrived @ 6:00 pm immediately determined MSHA Rpt'l. Called MSHA @ 6:10 pm. Fire was contained to this structure and extinguished.

Fire reported by Cont. @ 7:50 am involving beltline & oil tank being salvaged. Safety arrived @ 8:20 am. Determined to be MSHA Rpt'l @ 8:25 am, called MSHA @ 8:35 am. Fire contained/extinguised, damage contained to belt & tank. Called MSHA @ 1:35p.m. to report fire out. Surface being salvaged by JB Gordon Construction.
