EE was walking beside off side of roof bolter slipped on rock and fell, lt. foreman landed on roof plate cutting forearm.
Roof Fall 20 south section 23-bk 7/8 to 8/9 crosscut at spad 4044 #8 entry. Approx. 70' l. x 17' w. x 5.5' h.
Employee was hit in face with bolter wrench. Employee had just finished put in roof bolt. Had wrench in hand left foot jack down. Canopy hit wrench, struck employee in face.
Employee was using sledge hammer pounding gear on an belt reducer. Piece of metal went under safety glasses hit him in eye.

Employee, parked s/c on grade at 16-Block #3 Entry Spad #3868. S/c slid approx. 3' pinning oper. between s/c & rib, employee had set park brake and got off machine to move roof bolter cable.

EE was installing roof bolt, had hand on bolt hit rotation. Twisting wrist in #6-7 cross cut.
Stepped on piece of curtain and twisted ankle.
Employee was running roof bolter on 5G-Section 40-Blk in cross-cut 2-1, while installing roof bolt piece of rock fell between canopy & rib. Rock bounced off rib striking employee in lower back. Rock was approx. 12"W x 18"L x 3" Thick.
Swinging a sledge hammer and twisted Rt. shoulder #6 entry.
Employee was lifting belt structure and felt pain in right lower stomach
Employee operating diesel scoop. Ran over larger rock scoop raised up quickly, employee hit heat, knocked off hard hat and then head hit a steel strap (edge), cut top of head requiring stitches.
EE was in #3 face, piece of rock fell hitting him in the nose. Cut across bridge of nose.

Employee was working in 4 entry near face on 14-10AA miner. Putting cat pads together when cat pad slipped out of hand mashing finger between cat pad and pan of machine.
Emplyee's foot sticking out of mantrip. Pinched left foot between rib and mantrip. Zone 21-block breaking 3 toes.
Hanging line curtain in #8-Entry on 5-H Section. Bent down on knee's (right knee), went down on part of oil can, went under knee pad and cut employee across the knee.
Employee was working on 5-M section; piece of roof rock fall striking him on upper right leg.
Employee sliped on ice in yard. Fell on left wrist, breaking wrist.

EE was pushing on man trip (2-person ride) in 6 entry Blk 24 5G section. On hill put fingers between bar guard and battery case, battery slid back mashing 2 fingers.
Roof fall in #4 entry approximately 150' inby fan house. 8' H x 30' L x 6' W.

Roof fall E-Mains #2 Entry between 33+34 blk. Near station spad 737. 50'L x 6'H x 17'W.
Employee riding in mantrip. Left foot was extended outside of compartment. Mantrip close to rib, mashed left foot between rib & mantrip. Fractured big toe on left foot.
Backing up mantrip, tire hit rib and steering wheel spun around. Knob on steering wheel hit index finger on right hand.
Employee was straightening roof bolt to put in drill, pushed feed lever up & mashed 4th finger on rt. hand between drill head & plate.
Roof fall, E-1 submains, 6 to 7 xcut row 31 blk. Near station #948. 40'L x 5'H x 16'W.
Putting flights in feeder, prying chain to get flight put in. Bar slipped off edge and arm down and self. NOTE: Employee had surgery on 2-24-09. This was first day missed for injury.
Miner operator taking cut from 7-1X-cut, roof fell between rib and bolt. 44" long, 21" wide and 3" thick, striking EE on upper left back and left shoulder.

Employee was carrying a 4" water tree (for end of fresh water line) during belt move. He fell causing pipe to strike his left index finger.
Employee operating dual boom bolter. While running roof bolter, dropped head on left foot. Smashed on top of metatarsal. Broke bone in top of left foot.
EE riding in mantrip, right foot was extended outside compartment. Mantrip hit rib opposite machine operator. Right foot caught between rib and side of mantrip. Smashed metatarsal and cut boot. The right 3rd and 4th toes caught and cut at 1st joint.
Employee was resting left arm on top of bolter ATRS, started bolter and ATRS raised to the roof. Left arm was crushed between roof & top of ATRS. Broken left arm above elbow.
Roof fall 6'H x 18W x 20L C-Mains #5 Entry in Spad 334.
Employee slipped on ice going into outside shop. Hit shoulder on door jam. Tore muscle over right shoulder. Employee continued to work until 4-2-07.
EE was using a long lever bar to gain mechanical advatage to turn over a belt scrapper. The bar slipped, striking the EE in the jaw.
Miner operator turning left hand side cut, remote control miner. Roof rock 6" thick x 32" x 36" fell striking the employee on head and back.
