Loading 8" x 10" x 11" H-beam onto a flat bed trailer with fork lift. The injured attached a chain to the beam to reposition it on the trailer. When the fork lift raised off the bed, the chain slipped causing the beam to roll hitting pinning ee right hand between the beam and trailer resulting in an amputation of the tip of the middle and index fingers.

The employee was operating a D-10 bulldozer pushing coal on the D Bench coal stock pile west to east across feeder A. While pushing coal across the feeder the back end of the dozer fell approx. 8-feet into a void above Feeder A. The dozer was not able to pull out of the hole and was stuck. The employee was able to climb out of the dozer and walked off of the stockpile unhurt.

On 11/3/15 the mine operator was informed that the employee could no longer work at full duty due to a partial tear of the ACL in the left knee. On 10/22/15 the employee jumped from a ladder as the ladder was struck by a 28-inch ventilation tube elbow which was pushed into the ladder by a rib roll.

The employee was helping to move the rack wheel cover from the cable trough to the shield toes when his finger was pinched between the cover and the leg check on the #11 shield.

During a weekly examination a roof fall between crosscut 48 and 49 of the #5 entry 2nd West Mains was discovered.

The employee was helping to move a piece of steel for a chute repair into position when his right middle finger was pinched between the new steel and the old piece.

The employee was operating the longwall shearer headgate drum by radio remote. The employee was located about five shields outby the drum when a piece of rock was thrown by the drum. The rock struck the employee on the thumb while he was holding the remote.

While the employee was holding a drill steel in the roof bolt hole the support mast for the panline roof bolter allowed the bolter to tip toward the longwall face. The steel pinched the employee's left hand against the bolter mast resulting in a small cut and a chip fracture of the ring finger knuckle.

On 8/7/14 the mine operator was informed that the employee is a Part 90 miner.

On 7/27/14 2-feet to 2.5 sagged onto the top of the longwall stage loader and prevented the loader from advancing normally. The area of sagging roof was approx. 4-feet wide by 70-feet long. The #3 entry of the B-15 headgate panel had been supported with 8-foot fully grouted resin bolts on 5-foot centers with steel roof mesh and cable truss bolts on 5-foot centers.

The employee was dropping ventilation from the roof for re-use. A piece of rib coal and roof rock fell behind the employee and knocked him down. The rib coal/roof rock was 2-foot thick 3-feet long and 2-feet high.

On July 15, 2014, the employee saw a Dr. and was not allowed to return to work due to an injury that occurred on July 5, 2014. On July 5, 2014 the employee was shoveling coal from a shuttle car with a broken conveyor chain. While shoveling, the employee struck a conveyor flight with the shovel and strained his left shoulder.

The employee was watering roadways. When she made a left turn from a crosscut to the entry the water car contacted the rib. The employee had her left foot outside of the cab line and her left ankle was fractured.

On 8/8/14 the employee was unable to return to work at full capacity due to an injury that occurred on 6/1/14. On 6/14/14 the employee attempted to insert a roof bolt into the drill hole and shove the bolt through the resin cartridge by hand. The action strained his right shoulder.

While installing roof bolts at the start of unsupported face friction from the roof bolter wrench and a steel roof mat ignited an accumulation of methane.

While tramming a D-10R dozer onto the Wash plant feed coal stockpile the employee attempted to travel past a drawn down hole to the coal feeder. The side of the hole failed and the dozer slid into the hole. The dozer rolled onto it's side at the bottom of the hole. The employee was not injured.

The employee was carrying small parts and trash off of the longwall face in the headgate shields and tripped walking from #2 to #1 shield.

On 6/6/14 the employee could not return to work due to a right shoulder injury that occurred on 5/2/14. The employee stated that as he was attempting to hang a ventilation tube tee the loose coal that he was standing on shifted and he lost his balance straining his shoulder.

While attempting to set a timber the employee was holding the timber. When the cap piece was struck with an axe a piece of roof rock 10-inches X 10-inches X 3-inches fell and struck the employee on the head and right shoulder.

The injured employee is employed by Colorado Lime Company and was injured at Bowie Resources LLC, Bowie No. 2 Mine with MSHA ID 0504591. The injured employee slipped on an icy patch covered by snow and pulled his left hamstring. The treating physician placed him off work. He has returned to work with a full release.

The employee was found laying on the mine floor and was unresponsive to crew members. No cause for the employee falling could be determined. The employee may have a medical. The employee appears to have concussion like symptoms. The employee has a full duty release 1/27/14. Further medical testing is indicated.

The employee attempted to pick up a bundle of roof bolt plates that had been spilled to the ground. A bundle of plates on top of the pile moved and pinched the employees left index finger. The employee was working his last shift before an extended leave of absence but was released to full duty.

The employee was setting a concrete block under a battery charger frame. The tool box on the charger fell off which overbalanced the charger. The charger pinched the employee's hand between the frame and the block.

On 10/10/13 the employee was placed on restricted duty (no lifting over 30-lbs) pending further medical examination. The employee reported that on 9/25/13 he traveled through a man door and strained his lower back.

On 8/1/13 the employee reported to the mine operator that while the employee was loading vent tube into a Ram car he felt a pain in his lower back. The extent of the injury is pending further investigation.

While arriving for work the employee attempted to walk down the walkway from the parking lot to the bathhouse. Slick soled shoes caused the employee to slip and fall.

The employee stated that while packing a roll of curtain down the #4 lower b slope he slipped on the floor and fell. The employee stated that he suffers back disk degeneration.

While attempting to hang a length of water pipe from the mine roof the ladder used by the employee tipped over. The employee fell and landed on the ladder.

On 1/3/14 the employee elected to have his left little finger removed. The finger was injured on 5/28/13 when the employee removed the U-bolts from the axle of a welder trailer and his hand was pinched between the trailer frame and the axle hub. The ligament damage to the finger has not healed properly

On 9/16/13 the employee had surgery to reduce a hernia that may have been started on 3/14/13 when the employee was lifting bags of Tek-Seal onto the pump for mixing.

On 3/15/13 the employee failed to report to work as a result of injuries that may have occurred on 3/10/13. The employee claims to have suffered a lower back strain while packing a crib block to load it into a pickup truck.

While drilling a roof bolt hole the bolter operator bent the starter steel. When the steel was removed from the roof the bent steel rotated into the operator striking him in the neck.

The dozer operator on shift was pushing coal to a stockpile reclaim feeder when the dozer became stuck on the slope into the feeder. Poor coal pushing practices created the unstable slope.

The employee was traveling outside in a mantrip. The mantrip traveled into a hole in the roadway. The employee was bounced off of the seat and onto his lunch bucket.

The employee attempted to start drilling a roof bolt hole and started rotation instead of thrust. The roof bolter wrench being used on the starter steel was bent which caused the wrench to whip into the employee's forearm.

While the employee was attempting to re-set a circuit breaker on a diesel skid steer a battery hauler struck the skid steer pinning the employee between the pieces of equipment

Standing on large rock on side of road. Lost footing and fell to the ground, resulting in dislocated elbow.

Cutting metal and hot slag got into his pant leg and down into left shoe burning foot.

Was removing a dryer chute in the prep plant using an air arc and a pry bar. The pry bar got stuck, employee used a hammer to hit the bar. When the bar sprung loose it hit him above the left eye causing a cut

Pulling bit up through rig table, bit caught edge of table, bounced back into the table wrench which then struck the employee holding wrench in the abdomen.

Lifted an oxygen bottle to put on his shoulder when he felt a pop and pain to the back of his left shoulder. Employee missed his scheduled shift of 8/10/12 for shoulder pain.

The employee stated that he had moved over to the center of the entry behind the miner to check his sights. A rib roll occurred inby of the last row of bolts and a piece traveled outby and struck the employee in the ankle.

The employee stepped out of the cab of a Wagner 20X shield hauler and then reached into the cab and operated the fork down function. The machine articulated with the employee in the hinge point.

Employee chipped his front tooth while he raised the drill steel to his mouth and blew through to see if it was plugged or not. The steel came in contact with his front tooth and chipped it.

The employee was engaged in building a seal. In attempting to cut a hole in a Kennedy panel he struck his right thumb with a hammer.

The employee was loading roof mats onto the bucket of a diesel scoop. When he attempted to move the bundle of mats it slid and pinched his right little finger against the scoop bucket.

In attempting to remove a shield from the recovery face the shield was lowered to the transport position. When the shield was lowered the side seal broke loose and fell. The side seal struck the employees right knee.

The employee was struck on the hard hat and shoulder by a piece of rib rock. After the end of the shift the employee suffered symptoms of a concussion.

On 3/1/12 the employee underwent surgery to reduce a hernia that appears to have occurred on 2/8/12 when the employee attempted to lift an engine cover into place on a diesel scoop.

While the employee was attempting to cut a piece of conveyor belt with a utility knife the knife slipped and cut the employees leg.
