Showing all 5Wet, muddy ground, employee was stepping up on to the crusher to work on the crusher, his foot slipped and he hit the left side of his head causing injury to left side of face and left eye. He did refuse medical care, but changed his mind, and was taken to dr around 9:10 am.
Employee was using 4" grinder on Excavator EX-3 in a tight area when he slipped and the grinder cut his forehead requiring six stiches.
A gust of wind blew dust in his eye.
Employee was assisting three co-workers change a bearing on the primary shaker. As they installed the counter weight, which was being held in place by a come-along, which in turned slipped causing employee to get his hand caught. Injury turned out to be minor contusion.
