While attempting to get into the cab of one of the trucks, he grabbed one of the handholds and stepped onto the running board and twisted his knee.

Two EEs were making a connection. One pulled the Kelly up while the other set the slip in the table. The joint was set lower than usual. The two tongs were installed. The driller engaged the break-out tong 2-3 times unsucessfully. They decided to use the make-up tong to break the joint. When he engaged the clutch the tong came around the derrick leg and broke victim's hand.

Employee slipped on unknown object near screen rack area. To brace the fall, employee reached for the top screen and lacerated his hand.

Employee was removing a bolt when the wrench slipped and struck him in the mouth.

While puling on a bar to line up an I-Beam, employee felt a strain in his right shoulder.

Was attempting to set down a gear box and pinched his finger between the landing and the gearbox.

Employee was ascending the stariway to the wash plant bridge, and felt a pop in his left knee. No swelling. This pain had been an issue several weeks prior to incident.

Employee getting out of the man lift and slipped on some sand - went to grab himself and according to employee "popped his right shoulder out of socket." Employee stated that this had happened before and he was able to pop back in place, did not want to go to docotr, however called next day and wanted to see a doctor.

Was changing flat tire on grease truck, the jack slipped side ways and caught employee's hand between the tire and the fender.

While cleaning around wet plant - something bit EE on right thigh.

Area posted DANGER Do not enter unstable ground. Employee walking across to berm to retrieve rope that stabilizes float pump in retaining pond. Settling of the ground caused major cracks on top of berm. Employee heel on edge of crack as he attempted to step over it. Dirt gave way and he fell into the crack up to groin. Complained of pain in wrist, elbow, thigh and abdomin.

While inspecting a walk around, employee slipped into ditch causing strain to right knee.

Operator was trying to turn off the main water supply at the crusher to change a valve. The operator was standing on top of pipe turning valve when his feet slipped. He fell into the hole that was dug under the pipe in order to gain access to the bolts that secure the pipe. He sprained left ankle - notified supervisor but continued to work a normal shift day.

EMPLOYEE WAS ASSISTING IN THE CHANGING OF BEARINGS ON A TAIL PULLEY, HE LEANED FORWARD AND HIS HAND SLIPPED CATCHING IT BETWEEN THE BEARING AND THE FRAME.
