As the employee was washing off the back end of the Launch Vehicle used for Highwall Mining, the employee was walking down a set of steps on the Launch Vehicle when the employee stepped on the water hose being used causing foot to roll over sideways.

While installing the next ADDCAR (belt car) in line while mining, the injured employee and another employee was attempting to connect a de-energized 995 volt line connector. During this process, the line connector was inadvertently dropped onto the injured employee's hand which struck against the ADDCAR being installed.

The injured employee was pulling on a section of 250 MCM power cable and connector on an Addcar being installed on the Launch Vehicle when EE felt a "pop" in the right shoulder. This incident was not reported until 7-4-17.

At approx. 9:15am, co-worker noticed ee slumped forward in the cab and told another ee about what had seen. An ee went to the loader to check on ee and immediately started CPR. While doing CPR, EMS was called and ee was removed from the cab. EE was then taken to the hospital where ee was pronounced dead of Natural Cause Cardiac Arrest.

The injured employee was rolling up a power cable for a stacker belt with a co-worker when EE turned back and proceeded to trip over one of the co-worker's feet. While falling to the ground, EE twisted right foot causing injury.

While attempting to free an air hose coupling from the spooling eye of the reel with a pair of pliers, the hose unexpectedly continued through the spooling device; pulling the employee's hand along with the hose before the pliers could be released. A finger of the left hand likely hit the spooling device or the air hose and caused the tip of the finger to be over-extended.

Employee was working on the launch vehicle, taking a coal sample. After filling up the coal sample bag he picked the bag up to carry it to the back of the launch vehicle. As he turned with the bag in his hand he experienced some pain in his lower back. This was diagnosed as a lumbar muscle strain.

A temporary employee who works for Custom Staffing reported that he had experienced some lower back pains. The continuous miner had backed up onto the launch vehicle and a power cable fell onto the no. 1 conveyor car. Employee bent over to move the cable off of the car when he felt a pain in his lower back.

Employee had bent down to insert a metal pin into the skid assembly of a highwall launch vehicle. He needed a hammer to finish installing the pin and had rose up to walk to a maintenance truck. He walked about 6 steps when he experienced a sharp pain in the calf of his left leg which has been diagnosed as a muscle strain/ with a partial tear.

EE was replacing a conveyor belt on a conveyor car. He had bent over to pull the belt by hand when a belt puller that was attached to the end of the belt fell back toward him and struck his two front teeth. On March 15, 2011 he received treatment at from a dentist and minor cosmetic treatment was performed to repair small chips on those two teeth.

Employee was attempting to connect two sequence plugs together when the plugs silpped apart. When this occurred one of the plugs struck him on the right front tooth,chipping that tooth.

Combs had assisted in connecting the power cables to two conveyor cars together and was attempting to lay those cables into the tray on the conveyor car. While doing this he experienced some pain in his abdomen which has been diagnosed as a muscle strain.

EE was helping pull the trailing cable of the continuous miner off of the cable reel in an effort to locate the reason that the circuit breaker was knocking. While doing this his foot became entangled in that trailing cable causing him to fall; and his head hit the ground. As the result the side shields on his safety glasses caused a laceration that required 7 sutures to close.

Employee was pulling water line for continuous miner off of reel when a small piece of metal entered his right eye. (Employee was unable to see an optometrist on the day that the injury occurred to have the metal removed. He did get the metal removed the next day and then returned to work his next scheduled shift.)

While employee was trying to unthaw an air compressor some air brake anti-freeze that was sprayed onto his pants legs was ignited. While running to put out the fire on his pants he fell and dislocated his left shoulder.

EE was preparing to exit the steps at the back of the launch vehicle to do a welding task on a hydraulic jack end. When his right foot contacted the frozen ground he twisted his right ankle, which has been diagnosed as a sprain.

The employee was assisting in the removal of a panel cover for a continuous mining machine. He was crossing the conveyor on a disabled continuous miner when he stepped on a piece of rubber belting and slipped. When he did this he twisted his left ankle which has been diagnosed as a sprain.

Employee had positioned his left leg over the pusher assembly trough in order to add grease to a belt alignment fitting. The pusher assembly flipper was advanced and his leg was pinched between the flipper and a conveyor car pusher block resulting into a fracture and muscle damage to his left leg.

Employee was using a utility knife to cut a piece of damaged belt off of a conveyor car when the knife slipped and struck his left thumb. This resulted into a laceraton that required 4 sutures to close.

Injured employee and another co-worker were using an 18 inch crescent wrench to tighten a hydraulic hose on a manifold. The wrench slipped and the employee felt a pain in his left shoulder. At this time the injury is diagnosed as a strain, but additional examinations are scheduled. The employee worked 5 days of restricted duty and the injury became lost time on 01/15/09.

The highwall mining unit had just been moved and efforts were being made to reassemble that unit. EE was working in an area where work was being done to reinstall the walkway extensions. Prior to the last piece of grating being installed EE stepped back and fell into the void, injuring his left shoulder.

As the loader wiggled the car to free up the pin, he lost his balance.EE put his hand on the car to steady himself,car pin dropped pinching the middle finger on his left hand.He was transported to the ER where he was treated for a cracked bone on the tip of his middle finger.

EE was walking behind the Launch Vehicle and around the stacker belt. When he stepped up onto the berm, to get around the stacker, he lost his footing. This resulted in him falling down the spoil slope and tumbling approximately 40 feet until he came to a stop on a stump and some logs at the bottom. EE received abrasions and contusions on his upper torso.

EE was making a belt splice on a conveyor car and needed a hammer while doing this task. As he attempted to get off of the car he slipped on some snow that was present there and in an attempt to catch his balance, his left hand struck the car's frame, resulting into a hairline fracture to a bone in that hand.

A hydraulic hose on the highwall continuous miner had ruptured and EE was in the process of assemblying a new one. While trying to secure one of the metal end connections into the hose, by striking the hose end against a wooden bench, his left hand struck the bench. This resulted into a laceration to his left little finger that required 12 sutures to close.

The crew was in the process of pulling a bad cutter motor out of the miner using a chain and the loader. EE was about 25 feet away when the chain broke and the broken link hit him above his right eye. He was taken to the hospital, where he received 4 stitches to close the wound. EE returned to work on his next scheduled shift and did not miss any work.

Employee was in the process of soldering a ground wire in a heater. He did not notice that his right pants leg was too close to a fuel oil space heater and his right pants leg caught on fire. This resulted in second degree burns on his right calf and slight burns to both hands.

EE had dismounted the launch and started to walk towards the cars. On his first step forward in a level area he stepped on a lump of frozen coal fines and his left knee twisted and popped. He fell to the ground and landed on his right side. He was diagnosed at the hospital with a sprained knee and returned to work the next day on restrictions.

Employee was in the process of moving supplies to a new staging area. A car pin was falling off the retriever lid and when he tried to replace it another pin rolled against the first and smashed his ring finger on his right hand. This resulted in a fracture at the tip on his ring finger.

EE was holding up the stacker cable to allow the loader to clean underneath. When the loader backed out, the material caught the cable and gave him a quick jerk. He did not leave work or seek medical attention. He worked until 12:00 pm on 1-3-07 and at that time, requested medical attention. He returned to work the next day 1-4-07 on restricted duty.

The loader oeprator thought that they were finished servicing and started to remove the grease tub. The grease hose was stretched by the loader and was caught on the bottom of the hand rail gate. When the grease hose broke loose it jerked employee, pulling him toward the gate and fracturing his left arm.

Employee was on the upper deck of the launch, traveling under the hose coupling on the air reel. He raised his head up too quickly and hit his hard hat on the coupling. This caused pain in his back and neck.

Employee was removing rocks from the conveyor car, when a rock broke apart creating sharp edges. When the rock broke it slipped in his hands, lacerating the middle finger on his left hand. He was trasported to the hospital and required sutures to close the cut.

EE was walking toward a rock on a stopped car belt. He hit his L/leg against the car push pin, falling/stepping onto the Belly Belt into Incline Belt & Transfer Chute and halfway up the Stacker Belt before outby belts stopped. He received a contusion to lower R/Hip & scratch to L/Left leg. This report had been filed on 7-11-06 by Price Mine Service MSHA Contractor ID# J355.

Injured employee was reaching down to pull up the car pin, when the loader pushed the car and smashed the tip of his right thumb between the pin handle and the inby car. He was transported to the Hospital, where they cleaned and stitched the open wound.

Employee was welding on the track loader bucket when a spark from welding came in behind his welding shield and glasses, striking him in the left eye. The metal was removed from employees' eye at the hospital with a magnet and he returned to work. He was given prescription antibiotic eye drops.

Employee was removing a loader fork bar. When he unhooked a 2" clevis he was using to pull the bar out the clevis swung down and pinched his left middle finger. This caused a bruise under the fingernail.

Employees were looking for an oil leak on in the miner pan jack area. In the process of troubleshooting the leak a hydraulic hose blew, spraying employee in the face. He was approximately 10' away and was wearing his safety glasses. He was taken to the hospital to have his eyes washed out and checked. Employee was given prescription eye drops.

Employee came to work with Dr's excuse & a prescription for Cecebrex & Vikaden. Foreman told employee he could not operate equipment while on Vikaden. Employee stated that he went to Dr. for pain in his shoulder, wrist and right foot. He also stated tha his right foot was still giving him problems. The Dr. wrote to not use right foot on pedal that Miner Operator has to use.
