After scaling and installing roof jacks, injured employee began drilling the roof to install rock bolts. This activity caused a rock to fall striking the injured employee on the hardhat and right thumb.

While installing split-set rock bolts and switching drill steel to the jackleg drill, a small rock of unknown weight fell, striking EE on the ring finger of his left hand. This action caused a laceration approx. 1" in length on the top of the finger that required sutures.

EE CLIMBED UPON TRUCK TO FIX THE HOOD. HOOD CAME OPEN, HITTING EE IN THE FOREHEAD & KNOCKING HIM OFF TRUCK TO THE GROUND (BRUISED KIDNEY & INJURED BACK). EE STATED HE WAS GOING HOME TO REST, BUT NEVER CAME BACK TO WORK. WE COULD NOT REACH HIM TO DETERMINE WHY HE WAS NOT WORKING. THIS WAS THE REASON THE REPORT IS LATE. (AS OF 8/31: WILL HAVE MRI IN NEAR FUTURE; WC CLAIM FILED.)

Injured worker's co-worker let loose of the chain & it wrapped around the drill pipe. The injured worker was not expecting this action & was standing closer to the drill pipe than authorized. The injured EE was to return to work not later than 7/24/05. He abandoned his job on 7/21/05 & we consider him to have returned to work & quit.

Employee stated that he twisted his right knee and hit his back on a rock when he fell walking away from a jackleg. He stated that he tripped on a rock and fell. No one witnessed the alleged injury and he left the jobsite without informing his supervisor. He does not limp or show any signs of distress. The accident investigation cannot substantiate or deny the employee's claim.
