After finishing breakfast break employee was playing a joke on another employee and started running in the parking lot beside the office, while running a dog came running upon the employee, employee jumped above the dog and landed without balance in his right leg and broke one bone in his leg.

Miner was walking down a conveyor passageway, apparently he didn't notice the end of the conveyor with a ladder to step down, he kept walking and fell approximatley 5 feet to the ground.

EE was walking back wards pulling a water hose when a skid steer loader that was backing up ran over his left leg causing fractures.

Miner was trying to clean a chute from a screen with a hammer. The hammer fell onto the working platform floor and EE jumped from about 3' to the floor. When he tried to get up, his knees could not stand his weight.

EE and one co-worker were attempting to change the loader attachment from bucket to forks. Bucket got stuck, so EE started hitting it with a hammer. Bucket fell on both EE's feet.

EE was assisting a co-worker to change machine bucket to fork attachment. The co-worker noticed the bucket was stuck, so he started hitting the bucket with a hammer. The bucket fell on both EE's feet, fracturing EE's left foot.

EE (company electrician) was working on the control box of the secondary cone crusher. While moving some wires he felt an explosion which resulted in a second degree burn on his left hand due to the flash created by the explosion.
