Tripped on a rock

Maintenance work being done when employee grabbed motor belt.

Working on machine when their hand was caught in the machine.

Employee slipped while grabbing tools from truck due to the muddy conditions.

Employee was cleaning deck when EE got up, EE hit EE's shoulder on the corner of the power screen frame.

Employee was walking up stairs when they felt a sharp pain on calf.

Employee was doing maintenance work on a wing pulley by using a sledgehammer when a small piece of metal hit their eye.

Was replacing a lagging on belt when the metal bar slipped from EE's hand and fell forward hitting EE's cheek.

Gloves were not worn.

Crushed finger while on a harness.

Employee was lifting a metal plate during daily maintenance.

Was climbing down from the skid steer, bucket has a vertical plate. Employee did not see it and stepped with right foot twisting EE's ankle.

Condition was wet and raining.

Accident involving loader. Cause of the injuries unknown at this time.

Miner was working with a piece of metal and while EE was handling it, the metal cut into EE's right palm. Required a doctor's visit.

Employee was on top of a hopper removing material when employee jumped off and landed 4 feet below, strained right knee.

Minor - Cutting conveyor belt and knife slipped.

Slipped, Fell and injured Right side of Knee & felt pull to Groin area

He was lifting 50 lbs. sacks and loading onto back of truck. After loading about 150 sacks and tossing into truck. He felt a sharp pain on lower back going down left leg.

He was working, stepped on a rock, slipped, fell and then couldn't walk. He injured the Left Knee.

Employee felt a sharp pain in the Groin area while he was working to remove a metal rod from a mobile crusher. Hernia

He was working with Drill Rock Machinery and the Drilling Hammer hit his foot, Fracturing his left foot.

Slip & Fall on walking surface, injured right knee, Meniscal Tear.

He was shoveling material out from machines and underneath. When he went under the machine and was shoveling he felt a pop on his lower back.

He was fixing machinery slipped and fell backward. He fell about 10 feet and landed on his heels. He injuried both heels, left knee and lower back.

Tightening bolts that were loose on the side of a cone. There was little space to move and when he reached and leaned over more and pulled back he put lost of pressure on his back and thats when his side and lower back hurt.

While working on machinery, left middle finger was caught in between two metal pieces, cutting the tip of the finger.

Employee lifted a barrel and felt pain to his umbilical area.

A rock about 4" fell from conveyor striking employee below hard hat on back side of head causing a small scratch.

Hydraulic hose spraying liquid, employee jumped back to avoid spray and landed on right knee.
