Employee painting at reclaim feeder; snake of unknown species bit employee on lower left leg through pants. Employee taken immediately to ER where he was given a tetnus shot, antibiotics, and pain meds and kept under observation for several hours. Bite never swelled/showed any signs of infection. Employee missed no time, returned to work on scheduled next morning shift.

While moving a garden sprayer containing naturalizer (a cleaner/degreaser)closer to the binder catch wagon, the hose came off while under pressure and sprayed the naturalizer in his eye. Was taken to Princeton Hospital ER where eye was washed out and he was given a prescription for eye drops and 2 pain pills. No time was lost.

Slipped on wet floor while using a squeegie to clean up spilled material, injured right shoulder trying to prevent fall. Employee reported injury, did not seek medical attention until October 14. 2006 when he was taken to hospital with shoulder pain. He has not missed any work hours due to this injury, as to date.

Employee was carrying repair parts across the catwalk on the Synfuel #1 to infeed crossover and missed the step down. Fell and hurt left knee. Emplyee did not initially miss any work, or go for medical treatment. Went to doctor on October 14,2005; doctor drained fluid from knee and put on light duty for 10 days.

Employee working on step ladder, missed step while descending, causing him to fall aproximately 4-5 feet. Received abrasion to the left leg below the knee. Employee was taken to emergency room. Debridement of skin at injury site, and application of dressing and bandages.

Installing guard, removed bolt, motor/speed reducer assembly rotated striking employee. Employee received fracture to left ankle and non displaced fracture of one vertebrae.

Electrcian erected ladder; extension 14' base 5.5' from wall. Man proceeded to climb ladder to tie off, when ladder slipped, causing man and ladder to fall apx. 7' to concrete floor. Mans hand was pinned between ladder & floor, servering rt thumb from hand. Man was wearing harness but had not yet reached top to tie off. First aid administered, ambulance required

EE WAS FILLING KEROENE HEATERS USED TO APPLY HEAT TO BELTS. THE SKIDSTEER LOADER W/FORK ATTACHMENT BEING USED TO RAISE KEROSENE TANK TO RAISED HEATERS. AFTER EE RAISED TANK INTO PLACE HIE EXIT ED SKIDSTEER LOADER. WHILE EXITING, HE STEPPED ON ICE, LOST BALANCE AND FELL INTO FORK ATTACHMENT STRIKING HIS RIB CAGE.

PORTABLE BAND SAW WAS BEING USED TO CUT A SHAFT INTO TWO PIECES, THE EE REACHED TO CATCH THE CUT SECTION OF SHAFT AND CAME INTO CONTACT WITH THE SAW BLADE. FIRST AID ADMINISTERED, EE COMPLETED SHIFT. EE WENT TO DR NEXT DAY. THE EE WAS WEARING GLOVES.
