VENTILATION - EE WAS WORKING IN THE #6 HEAD ON THE MINE SECTION OF THE COAL THAT WAS BEING MINED. HE WAS STANDING ON A LADDER WHEN THE LEG OF THE LADDER GAVE WAY. HE FELL OFF AND HIT THE GROUND ON HIS LEFT SIDE (SPRAINED/BRUISED ELBOW).
Replacing pump and water line in #95 c/c area in approximately 3 feet of water when right foot slipped between two water lines causing me to fall backwards and injurying right ankle, right knee and hit left shoulder and neck into belt structure.
