Operator was burned by hot coolant after removing the radiator cap

Back Strain caused by a slip.

Pulling stuck equipment out, back injury.

Employee was exiting the maintenance truck and as they put weight on their left foot on the step below, they slipped and landed on their right foot, turning their ankle.

The employee parked the 740 Cat Haul Truck on uneven ground. The Employee left the cab to perform some tasks by the wash plant. When getting back into the 740 Haul truck, the step was higher than normal because uneven ground under the ladder, and as EE climbed onto the trucks ladder, EE pulled a muscle in their left leg.

The employee was exiting the haul truck and missed the last step with foot. EE tried to catch themselves but fell and hit the ground.

The employee stated that while backing in to be loaded by excavator, the excavator operator lowered the bucket into the bed of the truck causing the truck to abruptly stop. The truck operator stated that's this caused pain in their lower back.

The employee was exiting the haul truck when EE bumped EE's knee on one of the steps.

Employee's finger was smashed when EE was returning the tire hook to its storage location.

Driver of a haul truck fell asleep, ran off the road, and turned truck on its side. The driver got a small cut on the head that required one stitch.

EE was moving some metal screens around in the back of pickup truck with the help of a vendor. Doing so, EE's finger got caught between two of them and caused the thin edge of the screen to cut into finger tip. It cut part of the nail so it required 2 stitches.

We are still unsure of what has happened. The employee is under Doctor's care until determination is made.

Employee was walking to piece of equipment at beginning of shift. Employee stepped in a dip in the road due to limited lighting.

Uneven ground near pit equipment.

Operator was travelling in unloaded CAT 740B haul truck. Hit a pot hole in haul road causing him to bounce in seat (whiplash action). Operator has had previous, non-work related issues with neck, which required him to miss time from work. Operator had immediate pain in neck.

Employee was clearing and pushing brush into a pile, when a tree with tension snapped back at dozer and broke window on right side of cab. Employee had cuts on right side of face and arm. Foreign object in eye.

Employee raised up under piece of equipment and struck his back.

Employee was working in a heavily wooded/grassy area.

Employee attempted to snatch/pull a wash down hose that was buried in run-off material.

Employee was attempting to mount the ladder on a CAT 740B haul truck. His right hand grabbed hold of rail with some grease on it. As he shifted his weight up on ladder his hand slipped and he fell from ladder. His weight came down on his left foot/ankle.

Rain soaked, rutted out haul road lead to vehicle hitting hole and causing the operator to bounce in his seat. By his own admission, he was not wearing his seatbelt. Operator was bounced up and came down hard on the seat. Caused lower back pain.

Employee was stepping from an elevation off his trackhoe and twisted ankle when left foot contacted pea gravel and stick.

Employee complained of back pain following a trip from mine to plant. Road was rough and he stated he hit a bump in road and shifted sideways in his seat. Said he immediately felt pain in his back.

Employee was operating a truck on wet, pothole filled roads. He encountered a hole in road that caused him to bounce in his seat. Seat did not have air adjusted properly and he bottomed out during bounce.

Employee was cleaning his hands with a shop rag that had slivers of metal in it. As he wiped grease from hands a sliver of metal from rag entered his right thumb at the joint.

EE was changing teeth on trackhoe. He was hitting the tooth with a hammer to knock off when a piece of metal chipped off and went into right arm.

Employee was washing the concrete slab off under the hopper when the primary belt started overloading. The wet pit run starting running over the side of the belt covering his feet in mud. He tried to move and his foot was stuck in the mud. When he twisted his body to move his foot did not move.

Employee was hammering on tooth on a machine bucket. Chip came off and entered his stomach.

Employee was welding on pipe and burnt his eyes from the welding flash.

Doing normal work duties, as the day when on he complained with his eye getting irritated and redness to the eye. He used first aid eye wash to treat at the time. Next morning right eye was swollen shut and he went to the doctor. The doctor did not find anything in his eye, but said he had infection in right eye. Treated with medication.

EE was working on log washer. He pulled the trash rubber out and turned to throw it away and his knee popped.

Employee was under the hopper washing the slab with a water hose, when a piece of mud fell off belt and hit him in the back.

Hitting a pen on a track hoe with a sledge hammer. A piece of the pen broke off and lodged in left shoulder

Slipped and fell on wet plywood while nailing a board onto a piece of chute resulting in twisting ankle and bumping head.

Doing maint. on plant EE walked up behind EE using grinder and was hit in the left knee with the grinder. This caused a laceration.

While fueling equipment, diesel fuel got into employee's eyes.

Pouring up oil from oil shed and hurt back when lifting oil container.

Working on ground clearing brush to burn and twisted L knee.

Spider bite on left arm while cleaning debris from under hopper.

Leaving wash plant area, stepped on loose rocks and fell onto right arm (elbow).

Insect bite arm causing swelling and pain

Stepped off of bobcat dozer onto root and twisted foot.

OPERATING DOZER OVER A MOUND OF DIRT CAUSING THE RIGHT ELBOW TO HIT FIRE EXTINGUISHER. CUT ELBOW AND CAUSING SWELLING OF R. ELBOW.

Doing maintenance on 35 ton Volvo truck, he was laying under truck loosening belly pan; belly pan fell on left shoulder.
