Operator was getting loaded at the trackhoe when a rock was dropped into the bed of his truck causing the employee to be shaken and jarred around inside the truck.

Employee was operating a 100 ton rock truck. After dumping a load, the bed came down and jarred the truck, causing the employee to be shook and injuring his neck.

Employee had just gotten off work, he was driving out of the mine on the haul road and was trying to pass two other employees and got off the road and hit an old gate post.

Employee was getting on his dozer at the beginning of his shift, when he slipped and fell between the push arm and track. He injured his left shoulder and left hip.

Employee was exiting his rock truck at the end of his shift. As he was climbing down the ladder his foot slipped causing him to fall approx. 10 feet to the ground. He landed on his left arm. Employee broke his left wrist.

Employee was operating 100 ton rock truck. When dumping, the truck blew a hydraulic line. The fluid sprayed on the engine causing a fire. When he exited the cab he burned his arms and head. He also hit his head on the way out of the cab causing a cut that required stitches.
