Haul Truck Driver lost control of truck as EE turned a corner. Truck veered left over a dirt berm and into a small ditch nose down and abruptly stopped, thrusting driver hard towards steering wheel/dashboard, possibly hitting head on cab frame. Driver was initially unconscious. EE was later diagnosed with broken ribs, punctured lung and a brain bleed.
The employee was injured while repairing/cleaning the Chute with a high volume of sand on employee's left leg.

Cutting a bearing with a grinder and a metal piece went into their right eye.

EE was using a water hose to wash out EE's haul truck. As EE was walking backwards without looking, EE tripped on a cement k-rail and fell over it, in the dirt on EE's back. EE did not report the injury until 10/18/21

Employee was bit by a black widow while working in an electrical cabinet
Employee was using a cutting torch to cut a steel beam, approx. 200 lbs., 5" wide, about 1 foot from the ground. As ee was cutting the beam, it shifted and fell across both feet. EE right 2nd toe was fractured and ee sustained contusions to both feet.
EE was climbing down a ladder from the belt press plant. EE was going too fast and slipped on one of the steps which caused EE to fall the distance of 6-7 steps onto a concrete floor. EE hit the back of their head, right knee and wrist.
EE was shoveling dirt away from a hopper using a wood handled shovel. EE was not using gloves that were provided to ee and a wood splinter went into hand which got infected because ee signed a form to waive medical treatment on the day of injury.
While shoveling dirt and mud, EE hit right hand knuckle on a motor bracket, causing a laceration.
Employee states that ee was reaching into the truck bed of personal vehicle to put tools away when ee felt a sharp pain in back
EE was washing out a pulley with a water hose and slipped on the wet concrete which caused ee to fall on right hip. EE felt pain in back.
Employee tripped and fell exiting a portable toilet.
During a routine inspection of the 988 Loader, employee bent down to reach the lever to open the cab, when EE felt a "pop" in lower back. After a short time, EE started feeling pain and numbing in legs.
Unknown -employee was diagnosed with Lymphoma and feels it may have been caused by exposure to diesel fuel over time. PLEASE NOTE: EMPLOYEE DID NOT REPORT INJURY UNTIL 05/09/2016.
EE was repairing the underflow pipe to the thickener tank, slipped on mud and twisted left knee. **PLEASE NOTE: EMPLOYER WAS NOT NOTIFIED OF INJURY UNTIL 05/08/2016
Co-worker was driving a 988H Loader on the outskirts of the property to avoid scrapers. He drove the tractor over a hill, bucket lowered, he did not notice a pickup truck driven by employee, coming towards him. The front left loader bucket, hit the left front of the truck, injuring him. The employee in the pickup was hurt when the steering wheel was pushed into him.
Three employees were replacing screens at our aggregate wash plant where soil and rocks are separated. While replacing the screens, a couple tools and a screen fell down to a lower level. EE went to the lower level to retrieve the fallen items. While he was down there, another screen, approx. 5 lbs. fell on him, hitting him in the face.

Driver ran over pot hole on onsite road which bottomed out seat contributing to lower back pain.
EE is an independent sales consultant for TPH. While consulting on the subject of flocculant equipment, he slipped on the wet concrete and banged his knee. The area was marked "slippery surface", "watch your step". EMT called and transported EE and then he was released. He will follow up with his primary doctor and notify us if there are any complications.
Employee operates a front end loader repeately and digs into hard face mountain. Drives over a rocky road. Injury occurred over time; not a specific occurrence.

Mechanic was moving a full 55 gallon drum on to pallet. When he lifted the drum his neck snapped and he felt pain in his back.
On ladder aprox 4' up working with torch removing piece of channel from conveyor. Stored energy caused channel to spring toward miner hitting him in face/knocked him off the ladder. Had on fall protection & was harnessed correctly. It was not far enough for lanyard catch him. Fell & hit his knee. Received small scratch face, contusion/abrasion to left knee/shoulder, sprain knee.
Miner reported on 4/25/08 that his knee began to hurt 4/15/08 while operating a haul truck. Using the retarted aggrevates his knee. Left patella sprain/legiment.
Miner was assisting in replacing a screen on the bottom deck of the base plant. Miner raised his head in a confined environment and hit his head on a bolt that was on the strap of a guard over a cross tube.
Miner was operating a TD-25 dozer that was stuck in the mud. He was in reverse and hooked by a tow chain to another dozer that was pulling him out. The tow chain that was used to free the stuck dozer broke and a link of the chain broke through the back window of the dozer and struck the miner on the shoulder
Miner was moving a compressor. He grabbed a metal bar to get more torque and the bar broke loose smashing his finger against the compressor.
