Employee was drilling out a bogey pin with a hand held drill. The drill got caught on a bur and caused the drill to shift and come out of his hand and strike his left knee. Employee was attempting to splice a miner car cable. The cables were twisted and not easily identified, he cut the wrong cable that was energized. Cause of injury wasn't identifying the proper cable that was locked out and tagged. Metal trailer tongue slipped off hitch, fingers were still on the torque, pinched fingers between tongue and crib block. Cause was not moving away after placing tongue on hitch. Setting up an hose to blow out filters, has he laid down the hose, the coupler came un-hooked striking him in the mouth (lip). Cause of this injury was not making sure coupler was fully coupled. Compliance with rules/regs was not a factor, personnel protective equip. not a factor, miner attitude and proficiency could be a factor, mining equip/systems not a factor.

Pin was being removing from a pusher block on the highwall miner launch using a pry bar. He squatted down under the launch, pulled on the pry bar and felt discomfort in his groin. Cause was poor judgement in the means of removal of the pin. Rule compliance not a factor, mine equipment not a factor, proper clothing not a factor and miner proficiency and attitude may be a factor.

Employee was directing an end loader towards himself to hook a strap to it for removal of a dolly that was used to move the hi-wall miner launch. Contributing factors were attempting to hook a 6' strap while standing in front of this moving machine and stationary dolly. Cause was human error/bad judgement by the victim and end loader opr.

Employee was greasing the boom of a Cat 988F endloader with an air greaser. When the grease jerk would not take any more grease, he attempted to pull off the grease hose and it ruptured in his hand. The line pressure injected grease into his thumb. The thumb had to be operated on to remove the grease. Lost time injury.

Employee was getting ready to install a roof bolt. He had to place headers in a void in the top above the strap and as he stepped down, he struck his forearm against the bolt plate, cutting his wrist. The laceration required 10 sutures to close the wound.

Employee was cleaning mud off a miner bit. A metal burr was protruding from bit and his left thumb caught on the burr and lacerated his thumb. Employee had to have four stitches and returned to work.

EE was attempting to put antifreeze in the "Gen Set". When he removed the cap from the tank, air pressure blew cap upward, lost my balance and fell striking the tank. Cause of this incident was the lack of awareness and not releasing pressure on tank. Compliance with rules & miner proficiency a factor, PPE, mining/systems not a factor. Filed due to a part 50 audit.

Employee was pulling on female cathead bracket on a highwall miner conveyor car. The hinge pin was rusted, so he pulled hard on the bracket lever to free the bracket. He felt pain when he jerked on the lever. Shoulder strain.

Employee was stepping from the launch pad of the high wall miner down onto the bumper of a service truck when his foot slipped off the bumper. He fell past the bumper striking his rib area under his right arm. He stated that it knocked the wind out of him. He was diagnosed with bruised ribs and given prescription pain meds by the doctor. He returned to work.

Employee was stepping down from a high wall miner conveyor car, stepped into a hole or depression on the pit bottom. His foot turned under and inward as he placed his full body weight on the ankle. He felt immediate pain in the ankle area. He was transported to the emergency room and diagnosed with a bone chip in the ankle. Employee is a lost time.

The miner was stepping down from the back of a MMU Bulk Truck #126. His foot slipped off the ICC bumper and fell to ground. The miner tried to cushion his fall by reaching with his right hand and broke his wrist.

EE was in an enclosed cab of an excavator, loading rocks into the bed of an articulated truck when the rocks accidentally fell from the bucket causing them to fracture. Random flying rocks crashed through the wind shield hitting ee in the knee. He was treated and released for severe bruising of his knee at trinity medical center .

Employee was attempting to use a conveyor car connecting pin to push mud out of the conveyor car coupler. As he was tapping the pin against the mud in the coupler hole, his finger struck against the pin keeper, lacerating the finger. Three stitches required to close wound.

At around 9:30 pm, a phone call was made to inform OhioAmerican Energy that a small fire could be seen at the Prep Plant. The mine superintendent and safety director then drove to the Prep Plant. Upon arrival, a small fire (approximately 6 feet x 6 feet) could be seen burning on the coal stockpile. A D11 dozer was utilized to spread out the coal and quickly put out the fire.

A high pressure hose blew under the front of the operator's cab on the 992C loader and oil blew back on the engine compartment. The oil ignited and started the loader on fire apparently burning out the emergency shut off. The 992C loader was totalled but no one was injured.

Employee was pulling a connecting pin on a conveyor car on the Addcar highwall miner sysytem. The pin was bound at the connecting point so the employee jerked on it to pull it free. When he jerked on it, he felt pain radiate across his lower back. Physician evaluation was a strained lower back.

Employee was dismounting the steps from the launch deck of the highwall miner unit. His right foot slipped from the last step causing his left foot to get momentarily caught on the step just above it. He felt pain in his left ankle. He was diagnosed with a tendon strain of the left ankle.

Driver was hauling coal from the pit at the mine via pit haulroad. While descending a grade, driver attempted to downshift to a lower gear. Driver missed the gear and noticed that he was drifting towards the road berm. He tried to correct his steering and at the same time the right front tire blew out. The truck upset onto the roadway and stopped.

During an inspection, a hot spot was discovered by 2 MSHA inspectors & OAEI's plant manager. There were no flames present but steam was rising from a small area on the impoundment. OAEI was not permitted to take any action to remediate the hot spot. Approx 50 minutes later, MSHA issued a "K" order. A plan had to be developed before any remediation could begin.

Employee was dumping overburden from an end dump. He did not allow all the material to slide from the bed before releasing the dump lever. The bed came down abruptly onto the frame stops and jarred the employee. Employee had his seat belt fastened. Lower back pain the next day.

Employee was pulling the slack on the power cable for the conveyor car on the Addcar Highwall Miner. He stepped onto the rail of the launch to pull the power cable however he placed his ankle between the hydraulic car pusher and conveyor car, allowing his ankle to be caught between the pusher and car. Fractures of the ankle, tissue damage.

While lining up pins for a stiff arm on a CAT D-10 dozer, the pry bar being used slipped and the miner fell foward striking his right ear and right jawbone on truck outrigger. Miner was given an icepak and said he would continue working. Miner did not seek medical attention until MAY 02, 2009. Miner did not miss any work between March 25, 2009 thru May 05, 2009.

Employee was doing a pre-op check of a D11 Cat dozer. He openned the door and stepped out of the dozer to check the hydraulic oil. As he was bending over, he felt a pain in his lower back. The pain got worse as time went on and he asked to have a doctor look at it. Lower back strain.

While attempting to free a frozen conveyor belt on a highwall miner car, employee pinched middle and ring fingers of right hand when he struck the belt with a 2" x 2" x 24" long piece of square tubing he was using as a hammer. Middle finger was bruised and ring finger was fractured.

Employee stated that he was pulling the pin that connects conveyor car to conveyor car on the highwall miner launch. He placed his right foot between the car and the car pusher. As he pulled the pin, the car pushed forward and pinned his foot between the pusher and the car.

EE was removing the cap from the cathead of the 4160 power cable at the generator set. He spun the cap loose but as he pulled on it, it abruptly released from the threads and struck him in the mouth. His front tooth was chipped by the cap. It will need repaired by a dentist.

Employee was picking up a large clevis. As he was reaching to support the clevis with his right hand, a metal sliver (burr) went through his glove and punctured his hand. The imbedded object had to be removed by a doctor.

Employee was helping another employee lift a rock from the conveyor car on the High Wall Miner. The other employee lost his grip on the rock and the rock dropped onto the frame of the car. The employee who was helping remove the rock, pinched his finger between the rock and frame of the car. The first digit of his ring finger was fractured.

Employee was openning the access door to the crusher to check for blockage. After removing the bolts on the upper section of the door, he started to swing the door open. The hinge pins abruptly broke and the door dropped to the floor, striking the employees foot. Employee's foot was fractured on the metatarsal area of the foot. Employee was wearing metatarsal boots.

Employee was placing power cable for a highwall miner conveyor car into the channel on the car. As he was aligning it, the cathead on the power cable pinched his right index finger between the cathead and the conveyor car frame. His finger was fractured.

Employee was operating a water truck and was walking to the pump when he lost his footing and fell. He jammed his shoulder upon impact with the ground. He initially felt only a slight tingling sensation in the shoulder, but upon arrival at his home, he was unable to elevate his arm.

Employee was helping another mechanic remove a transmission pump from an end dump truck. While removing bolts from the pump, the mechanic he was helping dropped a 1/2 inch rachet (slipped from his hand) and struck employee in the face. His face was lacerated by the tool.
