Showing all 19EE was removing a punch plate from the shaker tower when the plate moved unexpectedly pinching two middle fingers on right hand against the frame of the shaker.
Employee was operating a 988K loader feeding a stationary crushing and screening plant. The operator drove the loader over a large rock, adjacent to the muck pile, causing operator to lose control of the loader. This caused injury to the operator's back. The operator required off site medical treatment and time off per medical professional
Employee was using a cheater pipe on a wrench to loosen a nut. When the bolt came loose the employee smashed hand between the pipe and excavator bucket. The employee received 2 stitches from the incident.
Employee was working on Impact Crusher when rotor turned pinching right hand between blow bar and side of crusher.
Operator was exiting cab of 336F Excavator holding on to the handrail facing the inside of cab when both feet slipped off track feeling a jerking motion to EE's left shoulder. The tracks on the machine were damp with material build up.
When returning to the excavator to be loaded the Articulating Truck ran through a wet area of the haul road under repair causing the driver to bounce upward and landing back into EE's seat abruptly resulting in injuring EE's lower back.
Employee fell off ladder catching self with right hand.
Employee was removing a bearing race from screen housing with a hammer tapping the race causing a piece of the race to chip off striking employee in the left leg above knee. The foreign body embedded under the skin.
Employee was helping remove a barbwire fence and cut his finger on one of the barbs. The cut required 5 stitches and the employee was released and could report back to work.
EE ( Plant Operator ) was using a razor knife to cut a piece of rubber belting when the knife came in contact with his left thigh.
EE had just finished fueling the EQ. when he laid the hose/nozzle down to climb down from the machine. As he put the nozzle down he stepped on it and it slipped out from under his foot causing him to fall. EE fell to the ground and landed on his stomach.
Driver was getting out of truck and turned to walk back to unlock the tailgate when he slipped in the mud. On the way to the ground he reached up with his right hand and grabbed for the door handle, instead he grabbed the door jam and the door closed so hard on his hand to the point it actually latched shut. Causing a laceration to his ring finger and exposed the bone.
EE had the inspection door raised over the hydraulic tank in the upright position. A gust of wind blew the door down and struck him on the tip of his middle finger on left hand crushing and breaking the bone in the tip of his finger.
Heat Exhaustion - became dehydrated while working on a shot.
Pulling on troughing idler to replace it and smashed his thumb between the idler and horizontal brace. Employee was wearing gloves.
Improper lifting.
Working on pre-screen that was deenergized, removing mud buildup from the belt. The belt rolled and caught right hand (last two finger tips) between belt and roller. Worker had gloves on.
Employee was welding on a platform for a cone crusher, fell about 3 1/2 feet to ground while descending ladder. Ladder was inside platform between two catwalks. Employee lost his balance and fell striking and fracturing his coccyx either on angle iron or rock on ground.
