On 1/28/22 a Ready mix concrete truck descended down a 10% grade mine road when the brakes of the truck became compromised. The driver lost control and the truck tipped over. IE was a passenger in training and suffered a broken leg in the accident.

Showing all 5Working on replacing belts on crusher. A ratchet slipped off the bolt head, another bolt that had not been removed was sticking out below, employee's thumb struck it and severely lacerated below the nail.
On Feb 28 at about 6:30 the loader operator discovered a fatal vehicle accident on the south side of the mine pit. The vehicle crashed onto mine site from a public road.
Employee was helping change out troughing rollers on the crusher. One was standing on end, the wind gusted and blew it over onto employee's foot.
Employee states while holding tube while fellow employee hammered same with bowling pin, he was struck by pin.
