operator decided to adjust a conveyer belt, conveyers were shut down. Guards were removed and adjustment was made. Operator decided to make another adjustment but failed to shut down conveyers, during which operators shirt became entangled in another conveyer wing roller. Shirt dragged operator into belt and pulley causing lacerations to shoulder, arm, and abdomen.

The employee's assistance was needed to remove debris that was clogging the crusher, not allowing it to work properly. In the process of clearing the jam, a 25lb rock rolled down onto his hand pinning his fingers between the rocks. He realized his finger was injured and was taken to the emergency room.
