The incident happened in the dimensional cut shop. The worker was using a wet polisher to finish a slab. When done EE would take slab and stack it on edge on a pallet. The slabs shifted and several fell over landing on top of the worker's left foot. EE did have safety boots on.

Skid steer operator was getting off of employee's machine when employee stepped down employee's foot landed on a small stone which caused employee to twist employee's ankle.

Truck driver was strapping down employee's load and got too close to the edge of the truck when employee lost employee's balance and fell.

EMPLOYEE WAS PUTTING WIRE MESH ON A PALLET USING A STAPLE GUN ONE STAPLE WENT THROUGH THE WOOD INTO HIS FINGER.EMPLOYEE HAS BEEN TRAINED USING THIS TOOL.

Miner was leveling a dump truck took a misstep and wrenched his lower back.

The miner was working at a wet saw. He had a rock in his hand that weighed about 12 to 15 pounds. He then struck the rock with a 3-pound hammer. The rock split in half. One piece stayed in his hand ad the other hit miner on his right knee.

Work area has slurry. The miner was walking with board, slipped and came down on left knee. Refused medical attention until 7 days later.

Employee was working at the rock splitter cleaning between rollers and a rock came down the roller table crushing pinky finger on right hand.

Employee was working on a machine and fell off about 8' while doing maintenance saddled a sheet of metal hurt groin area left side.

Employee making pallets of stone bent over to pickup stone felt pull in lower back.

Employees was making Flagstone has a fracture on finger with a nearby cut will follow up at 3:45PM Sept. 24th 2012.
