The roof bolter operator was drilling a hole for a roof bolt when a thin piece of shale slid down the steel (4 1/2 inches by 9 inches, paper thin to 1/4 inch thick). Before the operator could stop the rotation, the shale broke loose hitting the operator on the tip of the nose. Stitches were placed in the nose and no work was missed.
Employee was walking across the feeder and felt a pop in EE's right knee. EE was seen by an orthopedic specialist on 4/23/26 and was given a full release with no restrictions. EE continued to work until 5/1/26 when EE was seen by another physician who gave restrictions preventing EE from performing EE's normal job.
Employee was sitting in shuttle car as the miner was loading them when a piece of metal struck them in the left arm. Employee was treated at the ER where the object was removed and they received two sutures to close the wound. Employee was given a full release with no restrictions to return to work their next shift.

Employee was installing a pin in the rib and as they tried to line the steel up with the chuck, their right index finger got caught between the two resulting in a laceration requiring five sutures. They received a full release with no restrictions and returned the same shift.

Employee was installing a roof bolt with a board and as they pushed it to the roof, the tip of right middle finger was caught between the board and the roof. They had surgery to repair the finger and was taken off work due to restrictions preventing them from performing their normal duties.
While trying to remove the outside roof bolt that had bent entering the roof strata during the bolting process, the operator used the boom to remove the roof bolt and start over. In doing so the operator swung in pulling the pin and a piece of roof strata down with the boom, catching their left hand between the tray and the roof strata. Started missing work on 2/6/2026
While watering the roadways they stopped the diesel tractor to check the sprays on the water cart. As they were walking to the back of the water cart, they were tripped up by a nylon band laying in the road causing them to fall and twist their right knee. They worked until a week before surgery. Started missing work on 03/30/2026.
Employee was loading glue onto the back of EE's bolter. When EE turned EE felt pain in EE's back. EE treated at the ER where EE was taken off work for back pain.
The employee picked up a sheet of 4x8 plywood when the wind caught it. Trying to hold on to the sheet it twisted causing the employee to tear the right bicep. Started missing work on 2/12/2026.
The Mechanic was assisting in changing the cats on a miner. While the mechanic was picking the cats up to pull slack towards the back of the miner it pinched the left pinky finger in the hinge point of the cats causing a laceration and receiving stitches. NO work was missed.
Employee had pulled a diesel golf cart into a crosscut with the back partially in the road when a mantrip struck the rear of it. EE was getting back on the golf cart at the time and the steering column struck EE in the groin area. EE received bruising and an abrasion to EE's scrotum. EE was taken off work the next day as EE was unable to perform EE's job due to restrictions.
Employee was walking while carrying a timber when EE fell. EE's left hand contacted sharp rocks on the mine floor resulting in lacerations requiring sutures. EE was given a full release to return EE's next shift with no restrictions.

Employee was leaving a unit on EE's golf cart. As EE was preparing to turn on the road, a mantrip turned striking the front of employee's golf cart causing EE to bump EE's left knee. EE continued to work until 2/10/2026 and had surgery on 2/11/2026 on EE's left knee and was taken off work due to not being able to perform EE's normal work duties.
The operator dozed off while driving a mantrip, causing the trip to hit the rib. The operator hit EE's left knee on the dash of the ride resulting in sutures to close a cut. No work was missed.
A passenger of a mantrip that hit the corner of a rib was dislodged from the seat and landed on the mine floor. EE was diagnosed with two stable vertebral compression fractures in the spine. Started missing work on 12/02/2025.

A roof fall occurred on the 4th southwest BL seal area. in #11 entry from spad 93+75 through spad 93+10. The fall measured approximately 85'x 19' x 8'. This fall will not be loaded out, and the area has additional support added.

The roof bolter was drilling a hole for EE's rib pin when a rock fell hitting the canopy swinging in and striking EE's right shoulder, lower back and EE's right knee. The rock pushed EE's knee into EE's boom causing a contusion and swelling to the knee. Started missing work on 10/17/2025.
Employee was operating a grease gun injecting grease into a fitting on the continuous miner to tighten the cats. The connection was stuck, while prying on the connection, it popped off and caused a laceration to EE's forearm. EE received sutures to EE's forearm, was given a full release without restrictions and performed EE's normal occupation.
The miner operator used EE's index finger to rub EE's right eye causing dirt from EE's glove to enter and scratch the eyeball. Seen the doctor on 10/15/2025 where medication eye drops were prescribed and no work was missed.

Employee was dropping the bolter cable as it was tramming. EE was unaware that EE had stepped between the bolter and the rib and as the pinner swung it struck EE's lower left leg resulting in a fracture. EE began missing work the following day 9/26/25.
