As the Safety Technician was walking past the power center, EE tripped over a cable causing EE to fall awkwardly on EE's right knee. Started missing work on 5/18/2026.
Employee was cutting a plastic band off of concrete block with a utility knife when it struck EE's left thigh causing a laceration. EE was treated at the ER where EE received two sutures and a full release with no restrictions to return EE's next shift.
The roof bolter operator was drilling a hole for a roof bolt when a thin piece of shale slid down the steel (4 1/2 inches by 9 inches, paper thin to 1/4 inch thick). Before the operator could stop the rotation, the shale broke loose hitting the operator on the tip of the nose. Stitches were placed in the nose and no work was missed.
Employee was walking across the feeder and felt a pop in EE's right knee. EE was seen by an orthopedic specialist on 4/23/26 and was given a full release with no restrictions. EE continued to work until 5/1/26 when EE was seen by another physician who gave restrictions preventing EE from performing EE's normal job.
Employee was sitting in shuttle car as the miner was loading them when a piece of metal struck them in the left arm. Employee was treated at the ER where the object was removed and they received two sutures to close the wound. Employee was given a full release with no restrictions to return to work their next shift.
The utility person was cutting the seal out of a 1-quart bottle of engine oil with a belt knife when they accidentally cut the end of their left thumb. 3 Stitches were required no work was missed.
Employee was using boom pressure on the roof bolter to get steels together when they moved suddenly striking right hand. Employee was treated at the ER where x-rays revealed a fracture. Employee was taken off work with restrictions preventing them from performing their normal duties.
Employee was stepping over a trailer tongue when they lost balance injuring right knee. EE was seen at the ER where they were taken off work as they were unable to perform their job without restrictions.
Employee was injured while working on a header. EE was using a pipe wrench on a steamboat jack, when the force made the pipe wrench break loose from the jack and caused EE's arms to extend abruptly and hit EE's left arm and left shoulder on the header framing. Started missing work on 04/14/2026.
Employee was installing a pin in the rib and as they tried to line the steel up with the chuck, their right index finger got caught between the two resulting in a laceration requiring five sutures. They received a full release with no restrictions and returned the same shift.
Employee was cutting a belt off of the blower motor on a roof bolter when the knife slipped causing a laceration of their left wrist. Employee was treated at the ER where they received four sutures and was given restriction of not bending their wrist for three days which prevented them from performing their normal duties.
Employee was carrying a diesel particulate filter when EE stepped awkwardly causing a jolt to EE's right hip. EE continued working until 4/1/26 when a follow up with a physician gave EE restrictions preventing EE from performing normal duties.
Employee was moving a shuttle car cable to the rib and as they threw it they felt a pain in right elbow. Employee was seen the following day by a physician and was taken off work as they were unable to perform their normal duties.
While using a tractor to pull a header to bottom, the operator drove over a head that had been dragged into the roadway, resulting in a jarring motion that caused low back pain. Started missing work on 2/26/2026.
Employee was assisting the mechanic making a car splice when the knife slipped causing EE to cut EE's left index finger. EE was treated at the ER where EE received 3 sutures and a full release with no restrictions to return EE's next shift.
Employee was installing a roof bolt with a board and as they pushed it to the roof, the tip of right middle finger was caught between the board and the roof. They had surgery to repair the finger and was taken off work due to restrictions preventing them from performing their normal duties.
While trying to remove the outside roof bolt that had bent entering the roof strata during the bolting process, the operator used the boom to remove the roof bolt and start over. In doing so the operator swung in pulling the pin and a piece of roof strata down with the boom, catching their left hand between the tray and the roof strata. Started missing work on 2/6/2026
While watering the roadways they stopped the diesel tractor to check the sprays on the water cart. As they were walking to the back of the water cart, they were tripped up by a nylon band laying in the road causing them to fall and twist their right knee. They worked until a week before surgery. Started missing work on 03/30/2026.
Employee was loading glue onto the back of EE's bolter. When EE turned EE felt pain in EE's back. EE treated at the ER where EE was taken off work for back pain.
The employee picked up a sheet of 4x8 plywood when the wind caught it. Trying to hold on to the sheet it twisted causing the employee to tear the right bicep. Started missing work on 2/12/2026.
The Mechanic was assisting in changing the cats on a miner. While the mechanic was picking the cats up to pull slack towards the back of the miner it pinched the left pinky finger in the hinge point of the cats causing a laceration and receiving stitches. NO work was missed.
Employee had pulled a diesel golf cart into a crosscut with the back partially in the road when a mantrip struck the rear of it. EE was getting back on the golf cart at the time and the steering column struck EE in the groin area. EE received bruising and an abrasion to EE's scrotum. EE was taken off work the next day as EE was unable to perform EE's job due to restrictions.
Employee was walking while carrying a timber when EE fell. EE's left hand contacted sharp rocks on the mine floor resulting in lacerations requiring sutures. EE was given a full release to return EE's next shift with no restrictions.
Employee tripped over some car cable at the section substation. This resulted in a fall and the employee hitting their nose on the corner of the sub station. They received a broken nose from this.
Employee was walking from shuttle car to help the miner move when they twisted an ankle and fell to the ground. EE was seen at the ER where x-rays were negative for a fracture but employee was taken off work due to pain preventing them from performing normal duties.
Employee was roof bolting when partner told them they were advancing to the next row. EE acknowledged them, but as the bolter trammed forward they reached for pins placing left foot under the bolter tire. X-rays were negative for a fracture and EE was given a full release. EE was taken off work the following day as they were having pain preventing them from performing normal job.
Employee was leaving a unit on EE's golf cart. As EE was preparing to turn on the road, a mantrip turned striking the front of employee's golf cart causing EE to bump EE's left knee. EE continued to work until 2/10/2026 and had surgery on 2/11/2026 on EE's left knee and was taken off work due to not being able to perform EE's normal work duties.
The operator dozed off while driving a mantrip, causing the trip to hit the rib. The operator hit EE's left knee on the dash of the ride resulting in sutures to close a cut. No work was missed.
A passenger of a mantrip that hit the corner of a rib was dislodged from the seat and landed on the mine floor. EE was diagnosed with two stable vertebral compression fractures in the spine. Started missing work on 12/02/2025.
Employee was raising a light bar with a crank handle to illuminate a dump site on the refuge pile when they felt pain in left shoulder. Employee started missing work on 1/15/2026.
When hanging a cable in the belt line during a power move, the employee felt a pop in left shoulder causing pain in an area that had been repaired before. They continued to work. Started missing work on 3/3/2026.
Employee was using a hammer to smash a Burndy connector to splice a lifeline together, when they missed and struck their left index finger with the hammer. Employee was seen at the ER where x-rays revealed a fracture to the tip of the finger. Employee was given a full release with no restrictions to return their next shift.
Employee was preparing a cable bolt for installation and as EE reached across EE's body to grab a pin plate, EE drug the plate across EE's right bicep causing a laceration requiring sutures. EE was treated at the ER and given a full release with no restrictions to return EE's following shift.
Employee was driving a shuttle car on an outby construction project when EE's bumper rode up on the miner bumper and as EE pulled away the car dropped causing EE to hit EE's head on the canopy. EE was taken to the ER where no physical injuries were observed but EE was taken off work as restrictions prevented EE from performing EE's normal job.
Employee was lifting a diesel particulate filter when EE felt pain in EE's right shoulder. A later MRI revealed a tear in EE's rotator cuff. EE continued to work until 1/14/25 when EE had surgery to repair the tear and was taken off work as EE was unable to perform EE's job without restriction.
Employee was helping unit mechanic make a car splice on shuttle car, when the utility knife slipped causing them to cut left index finger. Employee was seen by the mine nurse where they received 5 sutures and a full release. Returned to their normal job the same shift with no restrictions.
The mechanic was standing on the shuttle car fender that was laying on the ground. While they positioned themselves to fix the problem, they slipped on the side of the fender causing pain in left knee. Started missing work on 11/14/2025.
While lifting the framing at a header with right arm the belt foreman felt a burning sensation in right shoulder. Worked until surgery date. Started missing on 03/25/2026.
Employee was drilling a hole when steels became stuck. As they pulled them apart, one struck them in the nose. Employee was seen at the ER where x-rays showed a fracture. Employee was given a full release and continued working until 11/12/25 when seen again and given restrictions preventing them from performing their normal job.
A roof fall occurred on the 4th southwest BL seal area. in #11 entry from spad 93+75 through spad 93+10. The fall measured approximately 85'x 19' x 8'. This fall will not be loaded out, and the area has additional support added.
Roof bolter was reaching into pin tray to retrieve a steel when a small head came from the roof smashing the tip of ring finger on right hand. Resulted in a fracture and started missing work on 10/29/25.
Employee was using their foot to slide boards under the tongue of an RA and when they put their weight on their right leg, they felt their right knee pop. Continued working until 12/1/25 when taken off for surgery to repair their right knee.
Employee was helping the roof bolter load roof bolts from a scoop bucket. EE had their right hand resting on the scoop bucket when a piece of draw rock fell striking their hand causing a laceration requiring sutures. EE was taken off work beginning 10/21/25 as they were unable to perform normal duties without restriction.
The roof bolter was drilling a hole for EE's rib pin when a rock fell hitting the canopy swinging in and striking EE's right shoulder, lower back and EE's right knee. The rock pushed EE's knee into EE's boom causing a contusion and swelling to the knee. Started missing work on 10/17/2025.
Employee was operating a grease gun injecting grease into a fitting on the continuous miner to tighten the cats. The connection was stuck, while prying on the connection, it popped off and caused a laceration to EE's forearm. EE received sutures to EE's forearm, was given a full release without restrictions and performed EE's normal occupation.
The miner operator used EE's index finger to rub EE's right eye causing dirt from EE's glove to enter and scratch the eyeball. Seen the doctor on 10/15/2025 where medication eye drops were prescribed and no work was missed.
While crossing over a supply trailer the tractor operator slipped and fell onto the ground. The operator worked until needing surgery to repair the left knee. Started missing work on 11/26/2025.
while hanging the miner cable and water line that goes to the miner, the shuttle car operator strained EE's left shoulder. Started missing work on 10/03/2025 and returned to work on 10/08/2025.
The mechanic was setting on a golf cart when a thin piece of draw from the mine roof fell striking the mechanic in the scalp. Resulting in staples to close the wound. No work was missed.
Employee was dropping the bolter cable as it was tramming. EE was unaware that EE had stepped between the bolter and the rib and as the pinner swung it struck EE's lower left leg resulting in a fracture. EE began missing work the following day 9/26/25.
