Employee was replacing bolt on screen when counter weight pulley pulled bolt & employee's left hand onto pulley under belt. Employee was pulling bolt when injury occurred. Center finger on left hand received 3-4 stitches.

Showing all 3At 8 pm on 4/10/08 a section of highwall collapsed. The materials covered half of the 992 c loader a rock broke thru the windshield causing a laceration of the scalp and right hand.
The welder was cutting a piece from a plste of steel. The plate steel was on a A frame bracket laying down: two other 4x8x3 1/6 plates were upright in the stand. He then stood on the plate laying down, causing the stand to tilt and then the plates to fall over onto the welder. The welder then pushed the plates off.
