Injured person was helping get the hood down on a coal loader. He was pushing on the hood and backed off the loader falling to the ground. He broke his right ankle and right heel from the fall. (HE is not a West Virginia Mine Power employee, the accident did happen on Mine Powers property.)

Employee finished shift and went home and his eyes started burning went to the ER where he was seen and had his eyes washed out. The ER doctor stated this could have been a flash burn from the welder.
Mine Examiner found a fall at Break 16 in No. 9 Entry at spad no. 2858 on West Mains. Fall measured 18'x12'x6'. No travel blocked, no ventilation blocked, and no injury. Fall was dangered off.
EE walked up to roof bolter had his right hand on bolter tray when a small piece of rock fell striking his right index finger.
EE was hanging CM cable and sharp pain felt in right bicep.Scans revealed tear in tendon/muscle in right bicep.EE is going to physical therapy and surgery may be required.This report is filed late because individual whom normally filed 7000-1 reports was recently terminated and the person filing this report discovered on 7/15/2013 that it had not been reported.
During the weekly mine examination a roof fall was observed in an intersection located in the old 1-south sub-mains in No. 3 entry, one crosscut inby spad No. 3865. This is a non-active area of the mine. The fall measured approximately 18 feet wide by 40 long and 6 to 8 feet high. No travel blocked, no ventilation blocked and the fall will not be cleaned. Only breakers posts set.
Employee was bending roof bolts, he felt pain in his right shoulder. He was seen at the ER X-rays were taken and read Negative.

Employee was walking to the section ,he tripped over a belt head guard that was down in the mud. He was seen at med express where x-rays were taken and read negative.
Employee stated that as he turned with a crib block he felt a pain in shoulder and low back. Did not report until 2/25/13. Employee stated that took x-ray on 2/18/13 and was told had a minor fracture to L-1 but could continue to work.
While moving the roof bolting machine from the operator's deck the employee pinched his arm between the machine and coal rib.
Employee was hanging a brattice curtain behind the power center, he was standing on a crib block the crib block moved causing him to fall against the power center hitting his side and ribs. He was seen at the ER and xrays was taken and he was told he had a fractured rib.
Employee was unloading trash from a flat car and tripped on tongue of flat car. He was seen at the ER for a contusion to the leg.
Employee was up on the bed of the fuel truck,it was covered with snow,he slipped and fell off hitting the ground with his right hand,he was seen at the ER a x-ray was taken and was positive for a fracture to his right hand.
It appears that the employee was loosening a hose on a piece of equipment; the wrench slipped and hit him on a tooth. To date the tooth has not been repaired and no work missed due to the incident.
A roof fall was discovered by the mine examiner in the intersection of No. 8 entry approximately 800 feet outby the dumping point at spad No. 2857. No ventilation, travelway or workers were affected by the fall. Fall measured approximately 20'x 20'x 8'high. Fall area cribbed and dangered off. Mine is non-producing.
Employee had came outside to fill out fireboss books, there was snow and ice on the ground, he was walking and slipped and fell. He was seen at summersville ER,where a x-ray was taken ,he was told that there was a tear in his Achilles tendon and there was a bone chipped in his left ankle.

It appears that the employee picked up a tool bag off a ride; spun around and twisted knee. Employee has continued to work. It appears that an MRI showing a menisci tear and mild pain was conducted on 1/2/13. Management was not aware of this until 2/19/13.

The employees states that he was impacting bolts and the swivel on the impact swung around and caught his finger, tearing his glove and cutting the ring finger on his left hand. Employee received 4 stitches at a medical facility and was released to full duty the same day.

Employee was roof bolting, a rock fell from the top between roof bolts and the rib tearing down line curtain. Employee was moving the rock off the curtain when he said that he felt something pull in his lower back.

A re-write of the incident indicated that the employee was jacking a rail supply car on the track and the edge of the car struck the rib area. CT scan in April indicate old rib fracture. Comp.denied claim. Received similar medical treatment/off 48 days from Jan. 2012 through June 2012 (prior to incident). Continue to investigate claim as to work related.
