D11R operator contacted me by CB approx 11:20 PM stating that he had injured his back climbing on his dozer at worktime (5:30PM). He said the pain had worsened as the shift progressed. I took him to Whitesburg ARH. The x-ray was negative. They said he may have pulled muscle or strain. He was advised to see family doctor on April 7, 2009

EE stepped down from grease truck, in doing so he turned his ankle on a piece of frozen dirt.

Operator was climbing down from 988 loader when he missed the bottom ladder rung or step causing him to turn his ankle when his foot made contact with the ground. He was taken to the A.R.H. ER where it was determinged he had sprained his ankle. He returned to work on 10 Nov 08 without missing a shift.

EE informed me at 2100 that he had injured his back getting on his equipment (992G) at work time. I then took EE to ARH (ER) for drug test and whatever medical treatment was needed.

Employee stated he dismounted D10 to use bathroom and was climbing back on the tractor when he slipped injuring his knee. He had knee problems prior to accident.

Employee was checking oil on D10N when he lost his footing during heavy rain. The grab rail caused him to twist possibly causing strain or back injury.
