Worker was folding up a tracked screen plant to do some maintenance on. When folding up one of the side discharge conveyors they slipped and fell on their shoulder and face. The ground was frozen.

Showing all 3Employee was picking up blocks to move the crusher to a recycle project. Employee slid on ice and twisted knee. Did not fall down. Employee put 2 week notice in on February 28 2020. 3 weeks prior the employee went on dirtbike trip & came back to work with this same knee hurt. Doctors put employee on light duty. On 3/13/2020 employee's two week notice became final.
At Startup a rock got stuck in the tail pulley of the cone recycle conveyor. Machine was shut down and the employee was removing the rock. EE's fingers got pinched while EE was getting rock out. A cut on EE's pinky finger needed to get a stitch that we were not able to perform on site.
