Pushing bolt up with the bolter, ee was holding the bolt and it slipped out and the plate caught ee hand and cut the palm of ee left hand.
Batteries ran down (no power) on the mantrip vehicle used to transport 2 people (spad crew) to the surface. One of the occupants head/cap got wedged between the mine roof (30' coal hgt.) & the vehicle frame & when machine movement occurred slightly, the occupant received injuries. Diagnosed as concussion and neck muscle strain.
Put up first bolt second row in #11 face let jacks down to move to second bolt. Bolter shifted pinning left arm against rib breaking left arm about 4 inches below elbow.
Operating scoop got left hand in pinch point and mashed two fingers on left hand forefinger and little finger.
The accident occurred at 6:25 AM 12-3-11. the miner were hand rock dusting the outby area when the scoop bucket crushed the victim between the scoop bucket and roof. the coal was 34 inches visibility was poor.
EE was blast a rock with a hammer and the EE reached to grab the rock and his hand got hit while hammer was being swung to blast rock. Location of accident was at tailpiece.
Employee was tramming a buggy down traveling on #2 belt line and caught his head against the top. Injured his neck and left arm.
