Employee had finished his shift & was ready to leave site from parking lot. He remembers leaving his overshirt in work area & went back to get it. It had rained much of the day & the area he was walking through was wet. He slipped in a wet spot, caught himself with his left arm/hand/wrist. The result was he broke the tip of a bone in his wrist.

Maintenance employee had been using a 4.5-inch hand grinder for 30 minutes on a bull gear in the shop when a metallic sliver ricocheted inside the face shield, around his safety glasses and imbedded in his left eye. The sliver was removed by an optometrist and the employee returned to work during the same shift without restriction.
