Injured suffered a ruptured eardrum while at work, initially injury was thought to have been caused from sinus pressure. On 8/4/2014 injured was seen by a specialist who determined the cause of the rupture to have been caused by a foreign body entering the ear.
Injured was pinned between the movable roller and the take-up frame after putting a come-along on the roller to free it. The roller broke free and struck the injured knocking him to the ground and proceeding to run over him causing severe injuries.

A dozer caught on fire while pushing coal. The fire was not extinguished within 30 minutes and was reported immediately to MSHA.

A slab of mine roof fell striking him on his left side knocking him to the ground. The roof rock measured at its maximum distance 15 foot long x 40 inches wide x 8 inches thick. The mine roof looked normal and was about 7' 6" tall. The section of roof that fell out had a polished slickenside surface above it that was not visible after the area had been mined and roof bolted.
A shuttle car operator was hanging miner cable from the mine roof. As part of this task he stepped on a chunk of coal strained his knee. He went to a doctor for initial observation and on the follow-up visit the doctor pulled some clear fluid off of the knee. The mine was not aware of the fluid draw until 4-19-13.
Injured was watering a set of batteries and a cell imploded and debris struck the injured person in the face and left eye. Injured sustained a scratch on his eye and received a prescription for his injury, making this a reportable incident.

AN unintentional roof fall occurred on the 2A submain on the 39XC of the #5 entry which is the belt line. The fall was approximately 17 feet wide by 18 feet long and over 10 feet high. The intersection was dangered out and a rehabilitation plan was implemented for the area.
Injured was recovering cable with 2 other individuals, when injured reached up to cut a piece of tie-wire the scoop bucket came in contact with his left foot. Injured was taken to local hospital for x-rays where it was discovered he had 5 bones broke in his left foot.

Injured had been setting timbers along rib line at the 170XC of the Main South 3. The crew had moved a rock away from the rib line in order to install a timber, injured took a step backwards and stepped awkwardly on the rock. This caused him to lose his balance and twist his left ankle. X-rays revealed the ankle was fractured and injured is now off work.
Injured was servicing a Dozer, when the grease hose he was using developed two pin holes. The pressure on the hose through the holes pentrated the skin on injured right hand, allowing grease to be injected under the skin. Injured was taken to local hospital and had the grease surgically removed.

Injured had just returned to work on 01/09/2012 shift, from being off, was helping build a stopping and said he re-aggravated his back. The cause of injury is unknown still to this date, but injured has missed work after reporting that an injury had occurred making this a lost time accident.
