Truck Driver was dumping a trailer of bypro, Driver dumped on UNLEVEL ground causing the load to shift and turn the truck & trailer on its side. Driver was is in the cab of the semi at the time and was not wearing EE's seat belt. EE then fell into the gear shift causing injury to EE's rib and chest area.

Showing all 16While climbing ladder on grader, pain was felt in neck and middle back and spread to shoulder and down right arm. Fell off the ladder and landed on my back. It was a cold, snowy day at the time of this incident.
While installing a ripper tooth pin on a dozer a 1 centimeter metal fragment from a punch cut the operator on the right side of the head. The fragment was removed requiring one suture.
Mechanic was cutting a mud flap with a utility knife and it slipped causing the blade to puncture is left hand between his thumb and finger requiring 2 stitches.
While operator was replacing backup light bulb on a truck he dropped the bulb and it broke and while getting out from under back of truck he put his hand down on the broken bulb and cut his left hand palm.
While operator was climbing down the ladder of his Rock Truck his boot drug on one step and missed the next step causing him to fall approximately 3 feet to the ground.
After tying off a coal barge, miner turned around and stepped into the barge winch cable causing the cable to hit him in the face. The cable shoved his eye glasses into his nose and right eye. This caused an abrasion to his nose and right eye corneal.
The alledged injury is stated to be from repetitive trauma from operating a rock truck. No accident or incident report was filed until 5-13-10 with the company. The medical treatment seems to be limited to diagnostic x-rays and Chiropractic Adjustments.
While installing a starter on a Cat 777C rock truck, the mechanic lost his grip and the starter slipped, pinching his finger between the starter and the engine. His middle finger on his right hand was pinched, requiring 2 stitches.
Mechanic was using service truck hoist to set hoist cylinder for CAT 777 Rock Truck. Finger was caught between two pieces, resulting in laceration, requiring two stitches.
While tightening bolts 988-12 the socket being used broke, which cut the mechanic's left hand, resulting in two sutures.

Truck driver was hooking lines onto trailer behind tractor cab. Started to dismount tractor deck, missed step and fell on left shoulder. Driver declined squad but agreed to be taken to ER to be checked out. Driver broke left collar bone, cracked or broke multiple ribs on left side and received scraps to left forearm.
A 2 Ton Come-along cable puller was being used to lower a pickup truck transmission to the ground. The chain or winch slipped and the handle struck the mechanic on the right side of the head and face. Four stitches were required to close the laceration on his eyebrow. Bruising was minor and did not require treatment.
Top rail of handrail was removed to provide service truck boom access to replace crusher parts. As the boom was being lowered at job completion victim stood and stepped back; striking middle rail of handrail. He fell approx 11 feet landing on his feet, and then right side. Injury occurred to Rt. ankle, Rt. elbow and Rt. shoulder.
Rock truck (R-8529) operator hit a bump in haul road in the 4 pit causing him to hit his head on the roof of the cab and cut the top of his head.
