On the I-6 retreat mining panel, a roof fall occurred in the #9 cross-cut of the #4 heading. The fall extended for approximately 60'. The area was originally supported with 8' torque-tension bolts spaced on a 4x4 pattern and 2 16' CC cable bolts installed every 8ft. Due to the location of the fall, the area was dangered off and supported, and the row of pillar blocks skipped.

Fall was discovered I-sub belt 9 crosscut. Fall was approx. 25ft long, 16ft wide, 42 inches high area was bolted with 8ft tension rebar and 16ft passive cable bolts area took weight and bolts broke causing fall. No ventilation, equipment or person affected.

was roof bolting on Fletcher twin boom bolter and had driver steelup and was adding pushed steel when due to misalignment the pusher steel did not seat to the driver steel and the pushed steel sprang back toward injured persons right hand and caused laceration that needed stitches.

Injured was loading belt structure on the surface for a future belt move. Injured was in motion with a piece of structure and caught left thumb between the piece the injured was holding and another stationary piece of belt structure that was lying beside the one the injured was holding. This caused a laceration of the lt. thumb that needed stitches.

Employee had finished putting in an outside roof bolt. When injured swung bolter head to inside direction, a rock believed to have been on top of canopy fell off hitting injured in left lower back causing injury.

Fall was discovered on the 10 Left 6 section in the #11XC going from #4 to #5 entry, including both intersections. The fall was approx. 90' long, 17' wide, and the height is undetermined. The area was bolted with 8' tensioned rebar bolts as permanent support and 16' CC cable bolts as supplemental support. The cause of the fall was due to abutment pressures from remnant pillars.

Employee was helping move a Distribution Box on J- section. Employee was looping a loop of cable onto a cable sled. Employee was on scoop operator side of scoop and sled. The sled hit a crib block and shifted pinning Employee right leg between cable sled and coal rib.

Injured was bolting on Fletcher twin boom Roof-Bolter when a piece of rock fell from the roof of mine and hit the injured in forehead and made a cut near left eyebrow. The cut needed 2 stitches to close wound.

During a power move on the 10 Left section, the employee was helping drop a Hi-Voltage cable from roof of the mine when the cable hanger end caught the injured persons right index finger causing a laceration which needed stitches. In the following days after stitches this wound became infected and employee began missing work as of 8/1/2017

Injured was carrying a bag of rock Dust into a power center location thru a door and felt stabbing pain in back. Emergency Room Dr. determined strained muscle in back and could return to regular job of Fire Boss on next shift. Injured returned to reg. job next shift and worked thru 6/20/17. Employee began missing work on 6/21/17 due to this injury.

Employee was loading bolter supplies onto Bolter when a rock (2ft. X 2ft. X 1 inch thick) fell from roof striking employee in left forearm making a 1 3/4" laceration that needed 5 stitches during treatment.

Fall occurred in the #4 Entry of the #7 crosscut of the 10left section due to geological conditions. Fall was Approx. 85 ft. long, 16ft. high, and 17ft. wide. Fall area was bolted to plan with screen and 8ft. tension rebar and 16ft. passive cable bolts. No people, equipment or ventilation effected. Alternate escape way was rerouted from area.

Employee was making way through an intersection on foot when ee ear came into contact with a piece of roof screen. The seam height was so that the employee had to walk around with head tilted to the right and ear pointed upwards. The screen caught the employees left ear causing a minor laceration.

A roof fall was discovered on the 11 Mains belt at approximately 7:30PM on December 26th, at the intersection of #12XC. The fall is approximately 36'L x 17'W x 20'H. The area was supported with 8' Torque-Tension bolts on 4' x 4' spacing with two (2) 16' cable bolts installed every other row. The fall was cleaned up and an arch was installed.

The area was bolted using 8' Torque-Tension roof bolts, installed on a 4' x 4' pattern, and two 16' cable bolts every other row. The fall area has been cribbed and dangered off.

Employee was roof bolting at the time of injury. The drill steel was not properly aligned with the drill chuck. The operator dropped the drill head to rotate the drill steel and it sprung back striking the left hand, fracturing thumb.

At approximately 2:56 am, a roof fall was discovered on the 11 Main belt line at #25 crosscut. The fall measured 18' wide x 60' long x 21' high. The area was bolted with 8' super twist primary roof bolts and 16' cable bolts. The fall was cleaned up and an arched canopy installed under the fall cavity.

At approximately 5:50am, a roof fall was discovered on the 3 Main belt line at between 17-18 crosscut. The fall measured 20' long x 14' wide x 4.5' high. The area was bolted with 6' super twist primary roof bolts and 12' PT cable bolts. The fall was cleaned up and steel beams set under fall area with cribs built on top of the steel beams.
