On 6/12/14 employee called in & said his back was hurting & couldn't work. He thought it was a pre existing injury of his sciatic nerve from 2008, he called back on 6/23/14 & said he wanted an accident report filled out due to jarring in a rock truck. He was off several days before reporting that he was injured at work.

Showing all 5Climbing down off of EX1100 Excavator, feet slipped out from under person, held body with both arms on ladder handrails. Pulled arms, shoulders & chest. Hanging 8-10" above ground, eased body off onto ground.
Repetitive use of hammers, wrenches, air guns and daily mechanic duties,
Service man was climbing off EX-1100 after putting oil in machine, lost his footing and fell approx 3' and landed on right side, hurting lower back.
Loader operator was loading 777 Cat truck with big rock. After rock was placed on truck he tried to push rock out of bed causing bed on truck to go up dumping rock out, and then the bed fell back down. Front wheels came off ground when rock came out of bed.
