Employee was walking to MCC trailer when employee felt something fly into right eye. Employee went to the first aid cabinet and grabbed some eyewash and flushed the eye. Employee could feel what ever was in there move around the eye. Employee was wearing proper PPE (Safety glasses) and believes it was an insect that entered eye.

Employee was replacing tail pulley bearing on conveyor. Bearing housing shattered when pried on resulting in a piece of metal flying into employees eye under safety glasses. Employee received minor scratch to left eye.

EE was stepping down onto catwalk after working on feeder hopper and as they stepped down they stepped on a rock. This action caused left ankle and knee to twist. EE notified manager but did not seek medical attention until 7/30/20. EE was given restrictions and referred to a specialist.

The employee was working in the locked out clarifier when the employee slipped and struck their back on the rake. The employee initially thought they were okay. We recorded the fall. The next workday the employee said their back hurt and the employee went to the clinic and was told they had strained back.

EE was cleaning material off of the conveyor. EE used a pipe wrench to turn the pulley. EE lost EE's balance and tripped and fell on the conveyor. EE cut EE's leg and was treated with 1st Aid. EE later complained of leg hurting and was taken to the clinic and left leg treated for a laceration. EE was given antibiotics and light duty.

EE was removing the cutting edge from a loader. The EE did not block out the edge and cut the bolts with a torch. The edge fell from an elevated position and struck the top of EE's foot.

Employee struck right index finger with a hammer while repairing a return roller in the maintenance shop. The injury resulted in 2 stitches and 10 days light duty.

The miner was dismounting a 400D John Deere Haul truck, when he caught his left foot on the top step. He did have ahold of the hand rail and was going down backward. He lost his footing and fell 6 feet to the ground landing on his back. He hit his right arm on a hydraulic cylinder, breaking his arm just above the wrist. There were no other injuries.

Miner was installing a metal ring on a water pump. He was using a 5 lb sledge hammer to drive the ring in. When he swung, the hammer missed the ring and smashed his R index finger between the ring and the hammer handle. He was wearing gloves. He cut his finger and cracked the bone on the end. Three stitches were required and a finger guard was placed on the finger.

On manlift to change roller on #2 belt - was raising basket when struck conveyor belt injuring forehead, bridge of nose & eyelid - nose & eyelid required stitches.

Miner was releasing a 1 1/2 ton cable come-a-long to lower a take-up bearing on #201 conveyor belt, when the rubber grip slipped off of the handle. The handle struck the miner on the right forearm causing a 1" cut. It took 5 stitches to close the wound. The miner returned to full duty with no restrictions on the same day.
