Employee was assigned to close a chute flop door. As the employee was positioned on a ladder to begin ratcheting with the chain ratchet, he slipped off the ladder and fell to the ground. Approx 4 foot fall, but in the process of falling he struck his left knee on metal flooring. The impact caused a fracture to his tibula bone near the knee.

Employee was cutting belt in order to make a splice in the belt. He was using a utility knife to cut the belt and he was positioned next to the rib and movement was limited. As he pulled the knife and it cut thru the belt he struck his left thigh with the blade causing a laceration to his leg that required 4 stitches to close properly.

Employee was installing a curtain on an 8 foot ladder, when as he was dismounting the ladder, it twisted and employee rode the ladder to the ground. He tried to catch himself with his right hand and suffered an injury to his wrist.

The injured was plastering a stopping when some liquid off of the plaster splashed in between his safety glasses and his face getting into his left eye. The injured was prescribed prescription eye drops.

Employee was cleaning 8 Mile No.4 belt, when he noticed a piece of rock in the off side of the belt structure. He alleged he used a nearby crossover to access the rock and removed it from the structure. As he was returning and dismounting the cross over his foot slipped on the step and he fell backwards and injured his lower back.

Employee was helping install belt structure when his leg gave way and employee started to experience pain. As he was driving home, he experienced pain again when pressing the brake pedal. A subsequent MRI showed no acute findings for the lower back area but noted degenerative changes. Note: This employee has a record of several occasions with experiencing back pain

Unintentional roof fall was found in a return off our 2 Left Mining section. The area where the fall occurred was bolted with 5 foot resin rods with draw rock pans as needed. The fall occurred in the 45th c-cut on 1 left panel between the #6 & #7 entries. Fall measure 20 feet wide at widest point and approx. 45 to 50 feet long and 5 foot high at the highest point.

When travelling the bleeder system identified as Old 8 Mile panel bleeder an unintentional roof fall was found in the #11 entry beginning at x-cut 23 and extending to x-cut #25. Fall was found at 2:00 PM and could not be determined when it had occurred as this is a weekly examined area. All access area has been dangered off.

Unintentional roof fall occurred in the 1 Left Panel X-cut 35, 72 feet inby spad #5985. It fell in the cross-cut between #6 and #7 entries. All access areas and adjoining entries have been re supported with cribs, rope bolts, torque tension bolts and fall is not planned to be cleaned up. Fall did not disrupt ventilation and entries beside the fall area can be travelled.

The injured was tramming the scoop in the #2 entry when he struck an indentation in the mine floor causing him to strike his shoulder in the operators compartment of the scoop.

Employee was bolting and attempted to install a roof mat between the roof and a t-3 strap. He lost his balance and fell off of the man-up provided on the bolt machine He dislocated his shoulder and fractured his humorous (upper arm). His shoulder was placed back into socket and his arm was placed in a sling. He was placed on light duty and allowed to return to work the next day.

Miner was operating the bolt machine and picked up a bundle of plates to step onto the man-up. He nearly missed the step but caught himself to regain balance. He hyper-extended his wrist. Later in the day he went to the ER, x-ray was negative. He continued to have soreness and was referred to an orthopedic on 1/6/15 where an x-ray revealed a fracture. He was placed on light duty.

The injured got the tip of his pinky finger caught between the concrete stopping blocks he was loading into a scoop bucket and the top edge of the scoop bucket. The injured was wearing metacarpal gloves at time of the incident.

Employee left office to attend the safety meeting. He forgot his notes and turned back striking his elbow on the handrail. A knot appeared on his elbow. He was diagnosed with Olecranon Bursitis. On 1-16-15, he had the knot removed.

After getting off of a ride the employee began to walking when he encountered rockdust and was having trouble seeing the bottom, he slipped on a piece of rock and twisted his right knee.

A Workers Compensation letter provided to the operator on 6-18-14 list a work loss date for 6-12-14 for the employee. A non-medical award determination was issued for Occupational Pneumoconiosis.

Employee was loading Kennedy overcast panels and a panel slid off the flat car striking his foot. He continued to work but the pain persisted after stepping in a hole at home while splitting fire wood. He went to the doctor on 5-11-14 and was diagnosed with a fractured fibula. He began missing work on 5-12-14 and notified the mine site on 5-23-14.

Contractor was plastering a stopping and the plaster splashed behind his safety glasses and into his eye. He flushed his eye and continued working but later his eye became irritated. He later went to the hospital. The hospital flushed his eye and prescribed Maxitrol Antibiotic drops.

Employee stated that while loading a communication and tracking node, he strained his back. Employee went to the dr on 2-28-14 and was placed off of work until 3-3-14. He did not report the incident to the mine site and turned in vacation days for his days lost. The mine site was notified by workers compensation on 3-10-14 that an accident had occurred.

Employee was removing a piece of draw rock from the top of the miner to work on the lights. While moving a rock, the employees fingers were struck by a piece of rock lying on the miner. His ring finger was cut requiring stitches and was fractured requiring a surgical pin to repair.

The employee was making a cable splice and started to crimp the burndy clamp. He was using a splice knocker to crimp the sleeves. When he struck the knocker, a piece of metal chipped off and went into his hand. The employee thought that he removed the metal but went to the doctor on 1-13-14 and underwent surgery to have it removed.

Employee was pulling on a piece of line curtain and strained his back. He went to the Dr and was given a pain medication and muscle relaxer. He rtw the next day but was not allowed to work due to the influence/impairment of the medication. Upon further consideration and this incident being deemed compensable, the incident is being reported as a lost time injury.

The injured picked up one side of a roll of typer to throw it out of the way and felt a pain in his left elbow. He went to his family chiropractor on 04/06/2013 who referred him to an orthopedic specialist on 04/19/2013. The orthopedist diagnosed him with a torn bicep tendon and scheduled him for surgery on 04/25/2013. The injured didn't start missing work until 04/25/2013.

Please be advised that the operator was notified on 5-7-14 by AIG Workers Comp Claims that the former employee has met the jurisdictional exposure requirements for non-medical acceptance, finding that the 10/15 year presumption that any respiratory impairment is caused by OP.

EE was directing the scoop operator pulling a belt in for the new section at the 3 mains-outby. The chain slipped and EE flagged the crew to check the chain. It was determined that the chain would stay on and the scoop continued to pull. The chain came off and hit EE in the face.

An unintentional roof fall occured in the face of the #6 entry on Rattlesnake section at #61 cross-cut on Rattlesnake #2 belt. The fall measured approximately 55 ft long x 20 ft wide x 8 ft high. The surrounding area was bolted with 4 ft resin bolts.

The injured was operating a scoop as it was being trammed off of a lowboy. The blocking kicked out causing the scoop to drop to the mine floor. The injured struck his head against the canopy of the scoop. The injured wasn't diagnosed until 08/30/2012.

The injured stepped between the frame of the roof bolter and the man-up platform while supplying roof bolt plates. This resulted in an muscle strain to his lower back. The injured wasn't diagnosed until Wednesday, July 25th.

The injured was using a chop saw to grind the sharp edges off a piece of key steel he just cut. In doing so, he cut his finger. The injured returned to work on his next scheduled shift.
