The area was originally supported with grouted rods. The area was dangered off with timbers and cribs. The mine is currently not producing and no employees were affected.

A roof fall was discovered on this date during a preshift exam. The fall measured 40'long, 6'-18' in width and was 3'-7' in thickness. The fall was located in the #3 entry in the Southwest Mains at crosscut #6. No one was present in the mine at the time of the fall. The mine is currently idle and ventilation was not affected. The fall will be cleaned up.
A roof fall was discovered on this date. The fall was located along the portal mains 1 crosscut inby spad # 2832 in the returns. The fall measured 18' wide by 50' long and was 8' in thickness. The fall has been dangered off. There were no persons affected and ventilation was not affected.
While hooking up a trailer to a lo-trac the employee asked the forks be raised. When the forks were raised, one fork came up quickly and came back down pinching the inside of the right leg between the fork and trailer causing a contusion. It was later determined the employee suffered an acl tear which required surgery. He began missing work on 8-22-2012
While employee was backing up the continuous miner he stopped to hang a loop of cable on the boom. While doing so, the boom of the continuous miner caught the employee's left leg against the coal rib resulting in a fractured leg.
EE stepped in mud and went to pull his foot out of mud and twisted his left knee.
Employee was traveling with KY State Inspector along belt line and when picking up draw rock that had fallen from the roof, rock slipped out of his hand causing a small laceration to the tip of his ring finger on his right hand.
EE was climbing the ladder to hang a chain fall when the ladder slid from underneath him and he fell to the floor hitting his head on a piece of metal that resulted in a laceration to the back of his head.
Employee was taking a guard off of a take-up and while pushing up on the guard, it hit the roof dislodging a piece of draw rock (16" x 16" x 2")that hit employee on the left leg and ankle.
Employee received a laceration to his left knee while cutting a tree down. He had notched one side of the tree and making the back cut, the tree started to and he was moving to a safe area when the saw hit his left knee.
While attempting to install a roof bolt, a piece of draw rock, slick sided and sharp, fell striking employee on his left hand causing a laceration to the middle finger requiring stitches.
Employee was helping splice belt and while attempting to square the edge of the belt, knife slipped and cut right leg above knee (thigh).

EE was cleaning (shoveling) the #9 head drive. EE had a shovel full of gob and twisted to throw gob on the belt line when he felt pain in the lower part of his back.
He was walking by another employee who was cutting a piece of metal. A few small pieces of metal went into his eye.
While retrieving High Line and other mining supplies in the Northwest Mains section of our mine that is in the process of being sealed, 6 falls and a large rib roll have been discovered. The falls did not interupt the ventilation in any of the bleeders. They range in size anywhere from 15' long, 20' wide to 70' long, 20' wide. All are at or above anchorage.
During a lease inspection a fall was discovered in a gob area located at spad 2097. The fall occurred in an intersection. The fall is approximately 5 feet high x 19 feet wide x 30 feet long. It was discovered at approximately 10:30 AM. The fall has been dangered off and this area is going to be sealed.

While traveling air course with KY inspector a roof fall was discovered at spad 2532. The fall is approxmately 30' x 19' x 6' high. Ventilation was not interupted. Fall was reported to MSHA field off and MSHA inspector was on site to inspect fall also. Fall located in bleeder area.
While traveling air course with KY inspector a roof fall was discovered at spad 2559. The fall is approximately 40' Long x 19' Wide x 6' High. Ventilation was not interupted. Fall was reported to MSHA field office and MSHA inspector was on site to inspect fall also. Fall located in bleeder area.
EE had finished bolting roof in the #3 heading. The Joy miner had a cutter motor bolt loose; the EE was helping the mechanic tighten the bolts using a ratchet multiplier. The multiplier slipped and the handle struck the EE's pinky finger on his right hand breaking the pinky in one joint.
Employee was positioned on his knees while operating roof drill. He turned his body to the right and felt pain in his right knee. Employee finished his and worked his next shift before missing.
Employee was opening air lock doors when one of the doors closed and caught him between the doors and door frame. The door pinned his right ankle under the left door. Employee received laceration to right ankle receiving 3 stitches. Employee returned to work on his next regular shift.

When traveling air course, a rock fall was discovered in the 1 Rt. Panel off of the 1 Left Submains Bleeder. The fall is approx 18' wide, 5' high and 30' long. The area has been cribbed and timbered off. The fall is at Spad 2461.
A roof fall was found on the 003 Active section at our mine The fall is located in an intersection break outby spad 2475 in the #4 entry. The area fall is approx. 70' long 20' wide and 5 1/2" high. The area had been dangered at timbered off.
At about 7:05 AM while making examinations, a fall was found in a bleeder system on Northwest Mains at Spad # 1333. The fall was approx 6 1/2' high X 18' wide X 20' long. Ventilation was ot inferupted and no one was hurt. This is a bleeder area, is located in #4 Entry.
