During crusher maintenance that involved rotating crusher blow bars, one employee was using a sledgehammer to dislodge the blow bar wedges. While striking the wedge, the employee missed and inadvertently struck the rotor. As a result, a metal fragment broke off and struck a nearby employee in the chest.

While performing a belt change, the employee crouched down to retrieve tools. As they attempted to stand, they applied pressure to their left knee, which resulted in a sharp pain. 5/2/2025 upgraded to Medical Aid. Grade 2 Sprain, requiring Physical Therapy

Operator reported that while driving the haul truck down the ramp into the pit, the truck slid sideways into a rut, causing a jarring motion that resulted in discomfort to EE's right wrist and lower back. EE was wearing EE's seatbelt at the time and had been holding onto the bar with EE's right hand.

While attempting to reattach a fallen guard on a crusher discharge tail pulley belt, an employee grabbed the guard through the openings and began to lift it into place. During this process, the guard at the back of the tail pulley was accidentally dislodged from its hook, resulting in a shearing injury to the employees left ring finger.

Employee used skid steer to take crusher blow bar pallets off of delivery truck. Once pallets were on ground employee tried sliding one blow bar to another skid to condense and stack skids for loading purposes onto a trailer to take back to main plant. When sliding blow bar, left pointer finger got pinched and caused a laceration to tip of finger.

Employee was tasked with moving a 100ft water hose located on a hill approximately 50ft over. Injured employee was located at top of the hill and assisting employee was at the bottom. Both employees grabbed their ends of the hose and tried to pull it over when the Injured Employee felt a sharp pain in lower back

Cat 775E was preparing to dump shot rock into crusher, when the operator heard a loud pop and the cab became fully engulfed in flames. Operator quickly exited cab of haul truck and had to jump approximately 8ft down. Emergency services were called immediately and were onsite in approximately 5 minutes, due to the nature of the fire it took approximately 3 hours to extinguish.

Operator was installing a new return roller when it slipped out of position and pinched the employee's finger between the roller and frame

Employee was splicing belt. EE was placing board underneath belt to help support for cutting. EE flipped the rubber belt over board and felt a shocking pain in finger tip on three lower fingers of right hand. Employee was feeling pins and needles in those fingers, employee then started feeling pain in their right thumb.

Employee had a tick bite.

At 1:00 pm the employee was cutting new shim plates for Jaw crusher, as EE was cutting with the Torch the sparks from the cutting caught on EE's left pant leg, it caught on fire and when our employee noticed EE quickly put it out with water.

While the employee was cutting a conveyor belt performing maintenance, the knife slipped and cut the inside of EE's left knee.

While repairing the steps to the water truck, the employee was attempting to hold a bracket in place, while using a hand drill. The employee was attempting to drill out old broken rivet to install new plate and bolts. While drilling out out the old rivet, the drill bit made contact with the employee's Left index finger nail cutting the nail.

Employee was using a metal hammer to drive collar on a shaft. While striking the hammer a piece of metal splintered off the hammer and struck the employee in the neck.

EE was cutting skirt rubber in a tight spot with a utility knife. EE was cutting towards self. Holding the rubber taught with EE's left hand, EE's right hand slipped and EE lost control of the knife and cut EE's forearm, right above where EE was wearing cut proof gloves - transported to emergency room where EE received 3 stitches.

Employee was showing another employee how to use quick disconnect on CAT skid steer. Used foot to move the lever and pant leg got caught. Employee was un-successful in catching self, EE lost EE's balance, and fell on top of disconnect piece protruding from the machine. Employee fell with EE's back hitting the piece.

Employee used the 980F as transport from C4, where employee was working, to the employee break room up top. At approximately 2:30am, the employee hit a bump & said the seat "bottomed out" & employee's back crimped. Employee initially thought employee was ok but, when employee tried to move a troughing roller, employee realized that employee couldn't do it because back hurt.

2 employees picking up a piece of steel (aprox. 100lb)-last of 8 that they were standing up. Come a long was used to assist employees with moving steel. Employee heard a "pop" in elbow. Pain through forearm. 3/2 went to Physicians and diag. with elbow strain and lateral epicondylitis - no restrict. Went to Riverside 3/9 - restrictions: <10 lb 4-6 hours, no push/pull, grip 1-2 hrs.

HDPE Pipe had fallen and workers were attempting to lift the pipe back up. Pipe became lodged behind angle iron and closer worker bent down to get a tool to try and pry the HDPE pipe loose. At that moment, the other worker, who was further away, managed to loosen the pipe and it flung backwards hitting the worker in the face. Safety glasses pushed in on face.

Employee was looking under the truck for an oil leak, when turned away stepped on a 3 to 4 inch rock and fell and fractured hand.

Employee was removing material from base of a pipe which was cut to be removed with a bin it was attached to. The weld broke at the top causing it to fall, hitting them on the shoulder. The employee was taken to be medically evaluated which stated that they had a contusion to the right shoulder and was able to return to work the next day with lifting restrictions for a week.

Employee was pulling the guard out of a conveyor with another employee for maintenance. The weight of the guard shifted pinching the employee's left thumb between the guard and conveyor. This resulted in a cut or tear of the skin next to the thumb nail. Required stitches. There was no damage to the thumb nail. Employee was wearing gloves at the time of the incident.

EMPLOYEE HAD GOTTEN DUST IN LEFT EYE WHILE WORKING IN SURGE TUNNEL

Employee was performing an end of shift inspection of the plant. He noticed what he thought was a splice in the belt had torn loose on the short sand stacker conveyor. He began to walk towards the belt and became distracted by the splice, he tripped over a raised sand spoil and fell forward and scraped his head on the conveyor/catwalk.

He took a wrench and tried to loosen slide bolts, after a few turns, employee attempted one last turn and the wrench slipped off the bolt. The pressure from the wrench hyper-extended his thumb causing it to break.

Lifting a quality control bucket and felt strain in right arm.

Approximately 10 inches of snow had fallen over the previous 24 hour period. Employee walked out of break room and slipped on an icy patch falling forward onto her arms.

Employee got out of loader to retrieve a piece of metal from the work area. Stepped on golf-ball size rock, causing his foot to slip. He fell onto his knee and felt a pop. Went to hospital and was diagnosed with a torn MCL.

Twisted left ankle on bottom step going up to plant. Went to Carle Clinic for medical services. Released to return to work with modified duties. No heavy lifting, no up and down steps, and wear ankle support if nessecary. No prescription drugs were given. Return visit on 1 May 2012.

Employee was replacing Universal Joint in Drive Shaft of service truck when Drive Shaft slipped. Employee reached out to catch the shifting Drive Shaft and received laceration on finger. Sutures, tetanus shot, and preventative anti-biotic administered.

Employee was kneeled down using hose to wash under conveyor belt. Strained lower back.

Knife slipped while cutting chute liner material

Employee was using wrench to adjust take up on tail pulley. Strained muscle while pulling on wrench.

While checking wheel lugs with torque wrench, wrench slipped, spun around and hit employee's face.

Lifting five-gallon bucket of rock to put into splitter for testing.

While helping change screens on screening units, tripped over cable that was attached to rollaway to pull chute away from screen, resulting in contusions to right hand and knee.
